摘 要:目的探讨基于信息-动机-行为技巧(IMB)模型的同伴支持教育在2型糖尿病(T2DM)患者自我管理中的应用效果。方法将130例T2DM患者随机分为对照组和研究组,各65例。对照组患者给予常规健康教育,研究组患者给予基于IMB模型的同伴支持教育,均干预6个月。干预前后,采用糖尿病自我效能量表(DSES)、糖尿病患者自我管理行为量表(SDSCA)评估两组患者自我效能和自我管理能力,采用葡萄糖氧化酶法和高压液相色谱法检测两组患者血糖水平[空腹血糖、糖化血红蛋白(HbA1c)]。结果研究组患者失访2例,完成研究63例;对照组患者失访3例,完成研究62例;两组脱落率比较,差异无统计学意义(P>0.05)。干预后,两组患者DSES评分、SDSCA各维度评分均较干预前升高,且研究组高于对照组(P<0.01)。干预后,两组患者空腹血糖、HbA1c水平均较干预前降低,且研究组低于对照组(P<0.01)。结论基于IMB模型的同伴支持教育能提高T2DM患者自我效能及自我管理能力,从而改善血糖控制状况。Objective To explore the application effect of peer support educat ion based on the information-motivation-behavioral skills(IMB)model in the self-management of patients with type 2 diabetes mellitus(T2DM).Methods A total of 130 T2DM patients were randomly divided into control and study group,with 65 cases in each.Controls received conventional health education and study groups did IMB-based peer support education on the basis of controls for 6 months.Before and after intervention self-efficacy and self-management ability of both groups were assessed respectively using the Diabetes Self-Efficacy Scale(DSES)and Summary of Diabetes Self-Care Activities(SDSCA),and blood glucose levels(fasting blood glucose[FBG]and glycosylated hemoglobin[HbA1c])of both groups detected adopting glucose oxidase method and high pressure liquid chromatography.Results Two cases were lost to follow up and 63 completed the study in study group;3 cases were lost to follow up and 62 completed the study in control group;intergroup difference in dropout rate wasn't statistically significant(P>0.05).After intervention the DSES scores and each dimension score of the SDSCA in both groups elevated compared with preintervention and those were higher in study than control group(P<0.01).After intervention FBG and HbA1c levels in both groups lowered compared with preintervention and those were lower in study than control group(P<0.01).Conclusion IMB-based peer support education can raise self-efficacy and self-management ability of T2DM patients,thereby improve blood sugar control.
