摘 要:目的:评价同质化床旁摄影对新生儿肺疾病诊断及临床动态时间窗治疗的应用效果。方法:回顾性分析楚雄彝族自治州人民医院2020年1月至2022年2月132例新生儿重症监护室患儿的295份床旁数字X射线摄影(DR)胸片资料,根据拍摄情况将其分为2组,比较2组影像图的病变描述与诊断分析。结果:同质化、高质量胸片在图像质量方面表现出较小的差异,对肺部病变影像容易做出相对客观的描述或明确的诊断,有利于评价复查前后同一病变部位的变化;与按个人习惯或经验摄片胸片比较,同质化床旁影像图像一致性、对比度良好情况及同部位同级支气管肺纹理显示情况更佳,复查前后无法动态评价情况和漏诊情况显著减少,差异有统计学意义(P<0.01)。结论:同质化床旁DR胸片是新生儿肺部疾病动态时间窗影像诊断与鉴别诊断的有效检查方法,有助于评价治疗效果,从而指导临床治疗并提高诊断准确性。Objective:To evaluate the application effectiveness of homogeneous bedside radiography in the diagnosis of pulmonary diseases in newborns and its role in dynamic time-window treatment in clinical practice.Methods:A retrospective analysis was conducted on 295 sets of bedside digital radiography(DR)chest images from 132 ill newborns in the neonatal intensive care unit of Chuxiong Yi Autonomous Prefecture People's Hospital from January 2020 to February 2022.The images were divided into two groups based on the shooting conditions,and the lesion descriptions and diagnostic analyses of the two groups were compared.Results:Homogeneous,high-quality chest images showed minimal differences in image quality.They facilitated relatively objective descriptions or clear diagnoses of pulmonary lesions,aiding in evaluating changes in the same lesion location between repeated examinations.Compared to chest images taken based on individual habits or experience,homogeneous bedside images showed better image consistency,contrast,and improved visibility of bronchial pulmonary markings in the same location and level.The ability to dynamically assess changes and reduce missed diagnoses between repeated examinations significantly improved,with statistical significance(P<0.01).Conclusion:Homogeneous bedside DR chest imaging is an effective diagnostic method for dynamic time-window imaging and differential diagnosis of pulmonary diseases in newborns.It contributes to evaluating treatment outcomes,guiding clinical interventions,and improving diagnostic accuracy.
