腹腔镜疝囊高位结扎术腹腔内缝合法与腹膜外结扎法治疗儿童腹股沟斜疝的临床疗效

摘　　要：目的比较腹腔镜疝囊高位结扎术腹腔内缝合法与腹膜外结扎法治疗儿童腹股沟斜疝的临床疗效。方法回顾性分析2019年1月到2022年8月北京和睦家京北妇儿医院收治的51例腹股沟斜疝患儿的临床资料,根据不同术式分为腹腔内缝合组(n=41)与腹膜外结扎组(n=10)。比较两组手术时间、术后住院时间、随访时间、并发症发生率及复发率。结果所有患儿均于腹腔镜下完成手术,无中转开腹病例。两组手术时间、术后住院时间及随访时间比较差异无统计学意义。腹腔内缝合组术后复发1例;腹膜外结扎组出现1例阴囊积液,两组患儿均未出现伤口感染、阴囊血肿、线结反应等其他并发症;两组术后并发症发生率及复发率比较差异无统计学意义。结论腹腔镜疝囊高位结扎术治疗儿童腹股沟斜疝,腹腔内缝合法和腹膜外结扎法均安全可行。Objective To compare the clinical efficacy of laparoscopic hernia sac high ligation with intraperitoneal suture and extraperitoneal li-gation in the treatment of oblique inguinal hernia in children.Methods The clinical data of 51 children with oblique inguinal hernia admitted to United Family Women's&Children's Hospital from January 2019 to August 2022 were retrospectively analyzed,they were divided into the intra-peritoneal suture group(n=41)and the extraperitoneal ligation group(n=10)according to different operation methods.The operation time,postoper-ative hospitalization time,follow-up time,incidence of postoperative complications and recurrence rate were compared between the two groups.Results All children completed the surgery successfully,with no cases of intermediate open method.There was no significant difference in opera-tion time,postoperative hospitalization time and follow-up time between the two groups.There was 1 case of recurrence in the intraperitoneal suture group;there was 1 case of scrotal effusion in the peritoneal ligation group;there were no other complications such as wound infection,scrotal hema-toma,and knot reaction in the two groups;there was no significant difference in the incidence of postoperative complications and recurrence rates be-tween the two groups.Conclusion Both intraperitoneal suture and extraperitoneal ligation are safe and feasible in the treatment of children with oblique inguinal hernia by laparoscopic hernia sac high ligation.
