基于黄元御一气周流理论浅析视网膜静脉阻塞

摘　　要：视网膜静脉阻塞是第二大致盲性视网膜血管疾病,眼底表现为静脉迂曲扩张,视网膜出血、渗出、棉绒斑,黄斑水肿,可导致无痛性视力急剧下降,甚至视力丧失。中医认为其基本病机是脉络瘀阻、血溢脉外而遮蔽神光,治疗当以活血化瘀贯穿始终。黄元御提出“左路木火升发、右路金水敛降、中焦土气斡旋”之“一气周流”理论,强调中气之要,重视培养中土、固护中气,此文基于“一气周流”理论,阐述视网膜静脉阻塞的病因病机为脾虚湿困、木气郁滞、肾阳亏虚,致气机运行不畅、阴阳失衡、瘀血阻滞、清阳不升发为目病,当从健脾、疏肝、温肾论治,根据静脉阻塞的临床特征、病程进展,从“一气周流”理论探析,为临床治疗视网膜静脉阻塞提供新思路。Retinal vein occlusion is the second most common blind retinal vascular disease,the fundus is manifested by vein tortuous expansion,retinal hemorrhage,exudation,cotton wool spots,macular edema,which can lead to painless sharp loss of vision,and even vision loss.Traditional Chinese medicine believes that the basic pathogenesis is vein blockage,blood overflow outside the veins and obscuring the divine light,and the treatment should be to activate blood circulation and remove stasis throughout.Huang Yuanyu put forward the theory of"general circulation of unitary qi"of"left wood fire rising,right golden water falling,middle jiao earth gas mediation",emphasizing the importance of medium qi,paying attention to cultivating middle earth,fixing and protecting medium qi.This paper is based on the theory of"general circulation of unitary qi",expounded the etiology of retinal vein occlusion as spleen deficiency and dampness,wood qi stagnation,kidney yang deficiency,qi machine running poorly,yin and yang imbalance,blood stasis,clear yang does not rise as eye disease,from the spleen,liver thinning,warm kidney treatment,according to the clinical characteristics of static obstruction,disease progression,from the theory of"general circulation of unitary qi".It provides new ideas for the clinical treatment of retinal vein occlusion.
