基于肝主疏泄探讨原发性肝癌的治则治法

摘　　要：原发性肝癌致死率高,治疗有效率低,中西医结合治疗有助于稳定病情,改善患者临床症状。中医理论中肝的主要生理功能为“主疏泄”,肝癌发病的病理基础也是从肝失疏泄开始,继而气滞血瘀、痰凝湿聚、邪郁化火生毒。因此治疗上提出原发性肝癌的论治应把握以通为要的原则,从“木郁达之”入手,行气活血,化痰利水,清热解毒,并配合柔养肝体,从而恢复“肝主疏泄”的正常状态,并以临床典型验案具体说明了攻补兼施、通利气血,使肝体阴用阳、维持正常功能的中医药治法。Primary liver cancer(PLC)has a high mortality rate and a low treatment efficiency.The combination of traditional Chinese and Western medicine treatment is helpful to stabilize the condition and improve the clinical symptoms of patients.According to the theory of traditional Chinese medicine,the main physiological function of the liver is controlling dispersion,and the onset of liver cancer is based on the dysfunction of liver.Afterwards the dysfunction leads to the stagnation of qi and blood,the agglomeration of dampness and phlegm,the generation of fire and cancer-toxin because of the gather of the evil spirit.Therefore this paper proposes the treatment of PLC should take dredging as the first principle,which is based on facilitating hepatic stagnation,and use the methods of promoting qi to activate blood,dissolving phlegm and removing dampness,clearing away heat and toxic,also nourishing liver to recover the function of liver to control dispersion.Meanwhile this paper showed a clinical typical case to illustrate the characteristic Chinese medical therapy of PLC by using the methods of supporting the vital energy and removing the accumulated evil simultaneously and dispelling the stagnation of qi and blood to maintain the normal function of liver.
