基于健脾通腑法辨治儿童功能性便秘临证思路

摘　　要：儿童功能性便秘为临床常见病,发病率高且易复发,影响患儿的生活质量和身心健康。小儿脾常不足导致气机升降失常,津液代谢输布异常,影响大肠传导形成便秘,治疗当以健脾通腑为法。饮食积滞所致便秘宜健脾理气、消食导滞;肝郁脾虚、气机郁滞所致便秘宜柔肝化郁、健脾行气;脾虚津亏便秘宜健脾生津、润肠通便;肺热壅滞所致便秘宜肺脾同治、补脾泻肺以通利肠腑。除药物治疗外还需注意患儿饮食、运动、排便习惯和心理状态的调整。Functional constipation in children is rare in clinical practice,which affects the quality of life and physical and mental health of children because of its high incidence and recurrence.Spleen deficiency in children make abnormality of qi movement and body fluid metabolism,affect large intestine conduction and induce constipation,which should be treated by invigorating spleen and removing obstruction of the fu-organs.Constipation caused by diet stagnation should be treated by invigorating the spleen and regulating qi,eliminating food and guiding stagnation,constipation caused by liver depression and spleen deficiency,qi stagnation,should be treated by softening liver and resolve depression,invigorating spleen and regulating qi,constipation caused by deficiency of spleen and body fluid should be treated by invigorating spleen and generating fluid,moistening intestines and defecating,constipation caused by lung heat stagnation should simultaneous treatment of lung and spleen,invigorating spleen and purging lung to removing obstruction of the fu-organs.In addition to drug treatment,attention should also be paid to the adjustment of diet,exercise,adjustment of defecation habits and mental state.
