基于虚瘀湿郁理论探讨中医治疗乳腺癌相关性淋巴水肿

摘　　要：此文介绍基于“虚、瘀、湿、郁”理论中医治疗乳腺癌相关性淋巴水肿的经验。正气不足,手术外伤,肝郁脾虚,湿瘀互结是乳腺癌相关性水肿发病的基础,此病病位在患侧上肢,局部辨证认为气虚血瘀,湿热内蕴,水湿停聚是此病发生的关键;整体辨证重在强调患者脏腑功能失调,分析得出肺、脾(胃)、肝三脏气机调节失常是局部气血失调的根本原因,即局部辨证包含于整体辨证之中。在针刺治疗方面,运用揿针,在患者健侧及远端取十二经脉的脾、肝、肺、心之原穴,任脉的关元、中脘,标本兼治,达到疏通经络气血、调节脏腑阴阳的目的。This article introduced the experience of treating breast cancer related lymphedema based on the theory of"deficiency,stasis,dampness and depression".Deficiency of vital energy,surgical trauma,liver depression and spleen deficiency,and the combination of dampness and blood stasis are the basis of breast cancer related edema.The disease is located in the upper limb of the affected side in traditional Chinese medicine.The local syndrome differentiation believes that the key to the occurrence of the disease is the deficiency of vital energy and blood stasis,the accumulation of damp and heat,and the stagnation of dampness.The overall differentiation of symptoms and signs emphasized the dysfunction of the patient's organs and organs.It is analyzed that the dysregulation of the qi mechanisms of the lungs,spleen(stomach),and liver is the fundamental cause of local qi and blood imbalance,that is,local differentiation is included in the overall differentiation of symptoms and signs.In terms of acupuncture treatment,lifting needles are used to take the original points of the spleen,liver,lung,and heart of the twelve meridians on the healthy side and distal end of the patient,as well as the Guanyuan(RN4)and Zhongwan(RN12)of the Ren meridians,to treat both the symptoms and symptoms,achieving the goal of dredging the meridians'qi and blood,and regulating the yin and yang of the organs.
