系统针刀疗法治疗腰椎间盘突出症临床经验

摘　　要：腰椎间盘突出症是纤维环破裂后髓核突出压迫神经根造成以腰腿痛为主要表现的疾病,是临床常见病、多发病。针刀治疗严重的、病变范围较广的腰椎间盘突出症尚在探索中,笔者在经筋、筋骨系统理论指导下,充分利用中医整体观念,结合近20年的系统针刀疗法治疗重症腰椎间盘突出症较为成熟的临床经验,形成了上海金山“系统针刀疗法”治疗腰椎间盘突出症的特色诊疗方法。此文从对针刀疗法的认识、常规针刀疗法入手,阐述系统针刀疗法治疗腰椎间盘突出症的理论基础、穴位探究、具体方案,并结合典型案例分析系统针刀疗法治疗腰椎间盘突出症的辨证思路和治疗特点,以期丰富腰椎间盘突出症的临床治疗方案。Lumbar intervertebral disc herniation is a common and frequently occurring disease mainly characterized by lumbar and leg pain caused by nucleus pulposus compression of nerve root after annulus fibrosus rupture.Acupotomy treatment of severe lumbar disc herniation with a wide range of lesions is still being explored.Under the guidance of the theory of tendon and sinew-bone system,the author makes full use of the holistic concept of traditional Chinese medicine,combined with the mature clinical experience of systematic acupotomy therapy in the treatment of severe lumbar disc herniation in the past 20 years.It has formed the characteristic diagnosis and treatment method of Shanghai Jinshan"systematic needle-knife therapy"for lumbar disc herniation.Starting with the understanding of acupotomy and conventional acupotomy,this paper expounded the theoretical basis,acupoint exploration and specific plans of systematic acupotomy for the treatment of lumbar disc herniation,and analyzed the dialectical thinking and treatment characteristics of systematic acupotomy for the treatment of lumbar disc herniation in combination with typical cases,in order to enrich the clinical treatment plans for lumbar disc herniation.
