糖脉康颗粒联合利拉鲁肽注射液治疗单用二甲双胍血糖控制不佳的老年2型糖尿病的临床研究

摘　　要：目的:观察糖脉康颗粒联合利拉鲁肽注射液治疗单用二甲双胍血糖控制不佳的老年2型糖尿病(T2DM)患者的临床疗效。方法:按照随机数字表法将152例单用二甲双胍血糖控制不佳的老年T2DM患者分为对照组(76例,均接受二甲双胍治疗,在此基础上接受利拉鲁肽注射液治疗)和研究组(76例,在对照组的基础上接受糖脉康颗粒)。对比两组疗效、血糖[空腹血糖(FBG)、2 h空腹血糖(2hPG)、葡萄糖化血红蛋白(HbA1c)]、血脂[三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)]、胰岛素抵抗[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞分泌功能指数(HOMA-β)]、血管内皮功能相关指标[内皮素-1(ET-1)、一氧化氮(NO)]。同时观察两组不良反应发生情况。结果:与对照组相比,研究组的临床总有效率更高(P<0.05)。与对照组治疗后相比,研究组ET-1、FBG、2hPG、HbA1c、TG、TC、LDL-C、HOMA-IR更低,HOMA-β、HDL-C、NO更高(P<0.05)。两组不良反应发生率对比无统计学差异(P>0.05)。结论:糖脉康颗粒联合利拉鲁肽注射液治疗单用二甲双胍血糖控制不佳的老年T2DM患者,可有效控制血糖血脂,改善胰岛素情况,同时还可调节血管内皮功能,安全性好,效果显著。Objective:To observe the clinical efficacy of tangmaikang granules combined with liraglutide injection in treatment of elderly patients with type 2 diabetes mellitus(T2DM)with poor blood glucose control by metformin alone.Methods:152 elderly patients with T2DM with poor blood glucose control by metformin alone were divided into control group(76 cases,all received metformin treatment,and received liraglutide injection on this basis)and study group(76 cases,received tangmaikang granules on the basis of control group)according to the random number table method.The efficacy,blood glucose[fasting blood glucose(FBG),2 h fasting blood glucose(2hPBG),glycosylated hemoglobin(HbA1c)],blood lipids[triglyceride(TG),high density lipoprotein cholesterol(HDL-C),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C)],insulin resistance[insulin resistance index(HOMA-IR),isletβcell secretion function index(HOMA-β)],vascular endothelial function related indexes[endothelin-1(ET-1),nitric oxide(NO)]were compared between two groups.Simultaneously observe the occurrence of adverse reactions in both groups.Results:Compared with control group,the total clinical effective rate in study group was higher(P<0.05).Compared with control group after treatment,ET-1,FBG,2hPG,HbA1c,TG,TC,LDL-C,HOMA-IR in study group were lower,HOMA-β,HDL-C,NO in study group were higher(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:Tangmaikang granules combined with liraglutide injection in elderly T2DM patients with poor blood glucose control by metformin alone,which can effectively control blood glucose and blood lipids,improve insulin,and regulate vascular endothelial function,with good safety and significant effect.
