心尖肥厚型心肌病患者超声心电图T波倒置与左心室肥厚特征的关系分析

摘　　要：目的探讨心尖肥厚型心肌病(AHCM)患者超声心电图T波倒置与左心室肥厚特征的关系。方法选取2018年1月至2020年10月辽宁中医药大学附属医院收治的180例心尖肥厚型心肌病患者作为研究对象,根据患者心尖壁增厚程度和类型分为典型AHCM组(n=119)与早期AHCM组(n=61),根据患者心尖肥厚范围分为单纯型AHCM组(n=67)和混合型AHCM组(n=113)。比较各组心电图特征、心尖厚度指标,并分析T波倒置与左心室肥厚特征的相关性。结果典型AHCM组TV5、TI均高于早期AHCM组,Tmax长于早期AHCM组,差异有统计学意义(P<0.05);单纯型AHCM组QRS时限延长、ST段下移发生率均低于混合型AHCM组,TV5、TI均低于混合型AHCM组,Tmax短于混合型AHCM组,差异有统计学意义(P<0.05);典型AHCM组与早期AHCM组、单纯型AHCM组与混合型AHCM组其他指标比较差异无统计学意义。典型AHCM组心尖与后壁基底部室壁厚度比值(ABR)大于早期AHCM组,心尖部最大室壁厚度(APWTmax)、心尖部平均室壁厚度(APWTmean)均厚于早期AHCM组,差异有统计学意义(P<0.05);单纯型AHCM组ABR小于混合型AHCM组,APWTmax、APWTmean均薄于混合型AHCM组,差异有统计学意义(P<0.05)。典型AHCM、早期AHCM和混合型AHCM患者中,APWTmax与Tmax、TV5均呈正相关(r>0,P<0.05),其余指标与T波各特征无明显相关性。结论单纯型AHCM患者APWTmax与超声心电图T波倒置深度密切相关,有利于患者进行影像学筛查和早期诊断。Objective To investigate the relationship between T wave inversion and left ventricular hypertrophy in patients with apical hypertro-phic cardiomyopathy(AHCM).Methods 180 patients with apical hypertrophic cardiomyopathy admitted to Affiliated Hospital of Liaoning Universi-ty of Traditional Chinese Medicine from January 2018 to October 2020 were selected as the research subjects,patients were divided into the typical AHCM group(n=119)and the early AHCM group(n=61)according to the degree and type of apical wall thickening,and the simple AHCM group(n=67)and the mixed AHCM group(n=113)according to the range of apical hypertrophy.The electrocardiogram characteristics and apical thickness in-dexes of patients in each group were compared,and the correlation between T wave inversion and left ventricular hypertrophy characteristics was ana-lyzed.Results TV5 and TI in the typical AHCM group were higher than those in the early AHCM group,and Tmax was longer than that in the early AHCM group,and the differences were statistically significant(P<0.05);the incidence of QRS duration prolongation and ST segment depression in the simple AHCM group was lower than that in the mixed AHCM group,TV5 and TI were lower than those in the mixed AHCM group,and Tmax was shorter than that in the mixed AHCM group,the differences were statistically significant(P<0.05);there were no significant differences between the other indexs between the typical AHCM group and the early AHCM group,the simple AHCM group and the mixed AHCM group.Apical to basal pos-terior wall thickness ratio(ABR)in the typical AHCM group was larger than that in the early AHCM group,the maximal apical wall thickness(AP-WTmax)and average apical wall thickness(APWTmean)were thicker than those in the early AHCM group,and the differences were statistically sig-nificant(P<0.05);the ABR in the simple AHCM group was smaller than that in the mixed AHCM group,and the APWTmax and APWTmean were thinner than those in the mixed AHCM group,the differences were statistically significant(P<0
