摘　　要：目的探讨高位复杂性肛周脓肿脓腔壁组织及外周血白细胞介素-17(IL-17)与肛周脓肿患者预后转归的关系。方法选取2018年5月至2021年5月中国科学院大学深圳医院收治的80例肛周脓肿和混合痔患者作为研究对象,根据疾病的不同分为肛周脓肿组与混合痔组,每组40例。所有患者均于本院行手术治疗,术后随访6个月,按治疗终点将肛周脓肿组患者分为两组,其中伤口愈合良好且无肛周脓肿或肿块复发者为恢复组(n=30),而肛周皮肤上出现充满脓液的单个或多个红色乳头状突起者为肛瘘组(n=10)。于术中留取病变组织标本,同时采集所有患者5 ml外周血,以免疫组织化学法检测病变组织标本内的IL-17阳性表达情况,以实时聚合酶链式反应(RT-PCR)法检测外周血IL-17 mRNA表达量;绘制ROC曲线,探究IL-17 mRNA表达量与肛周脓肿患者预后转归的关系。结果肛瘘组IL-17阳性表达率高于恢复组与混合痔组,差异有统计学意义(P<0.05);肛瘘组IL-17 mRNA表达量高于恢复组与混合痔组,差异有统计学意义(P<0.05);ROC结果显示,IL-17 mRNA预测肛周脓肿患者预后转归的关系较高,AUC为0.746,95%CI为0.746~1.000(P<0.001)。结论IL-17在肛周脓肿患者的肛周脓腔壁组织与外周血中呈高表达状态,其参与疾病的发生与发展,可预测肛周脓肿患者预后转归。Objective To investigate the relationship between the abscess wall tissue and peripheral blood interleukin-17(IL-17)of high complex perianal abscess and the prognosis of patients with perianal abscess.Methods A total of 80 patients with perianal abscesses and mixed hemorrhoids who were treated in Shenzhen Hospital,University of Chinese Academy of Sciences(Guangming)from May 2018 to May 2021 were selected as the research subjects,and they were divided into the perianal abscess group and the mixed hemorrhoids group according to the different diseases,with 40 cases in each group.All patients underwent surgical treatment in our hospital,and were followed up for 6 months after surgery.Patients with perianal abscess were divided into two groups according to the treatment endpoint,those with good wound healing and no perianal abscess or mass recurrence were divided into the recovery group(n=30),while those with single or multiple red papillae filled with pus on perianal skin were divided into the anal fistula group(n=10).Lesion tissue samples were collected during the operation,and 5 ml of peripheral blood was collected from all patients at the same time.The positive expression of IL-17 in the lesion tissue samples was detected by immunohistochemistry,and the peripheral blood was detected by real-time polymerase chain reaction(PT-PCR)method.IL-17 mRNA expression in blood;ROC was drawn to explore the relationship between IL-17 mRNA expression and prognosis of patients with perianal abscess.Results The positive expression rate of IL-17 in the anal fistula group was higher than that in the recovery group and the mixed hemorrhoid group,and the difference was statistically significant(P<0.05).The ROC results showed that IL-17 mRNA had a higher relationship with the prognosis of patients with perianal abscess,the AUC was 0.746,and the 95%CI was 0.746-1.000(P<0.001).Conclusion IL-17 is highly expressed in the perianal abscess wall tissue and peripheral blood of patients with perianal abscess,and is involved in the occurrence a
