摘　　要：目的评价LiCA800光激化学发光法在人类免疫缺陷病毒(HIV)抗体检测中的准确性。方法回顾性分析2021年5—10月于苏州大学附属第三医院送检的临床血液标本中,符合检测要求并通过LiCA800光激化学发光法检测HIV抗体,结果S/CO≥1的标本53例。以疾控中心反馈的WB结果为HIV抗体检测结果的金标准,分析LiCA800的检测结果。结果LiCA800检测有反应性结果(S/CO≥1)样本53例,采用酶联免疫吸附法(ELISA)和RT方法进行初筛试验:两种初筛方法均为阴性24例,对其余29例初筛呈反应性的样本进行WB抗体确证试验,WB条带阴性6例(11.32%),WB条带不确定7例(13.21%),WB条带阳性16例(30.19%)。WB条带阳性样本的S/CO值在30.31~223.97,S/CO值>100的病例占比75.00%。WB条带阳性样本S/CO值明显高于WB条带不确定、WB结果阴性和ELISA和RT法初筛阴性标本S/CO值,差异有统计学意义(P<0.05)。WB阳性结果中,gp160、gp120、p24出现率为100.00%,p55出现率较最低为43.75%。结论LiCA800检测血清HIV抗体时阳性符合率不高,仅为30.19%,S/CO值与血清HIV抗体WB确证结果相关,实验人员应对S/CO值和WB结果予以关注,避免纠纷。Objective To evaluate the accuracy of LiCA800 photoluminescence method in human immunodeficiency virus(HIV)antibody detec-tion.Methods Retrospective analysis was performed on 53 clinical blood samples submitted to the Third Affiliated Hospital of Soozhou University from May to October 2021,which met the detection requirements and detected HIV antibodies by LiCA800 photoactivated luminescence method,and the results were S/CO≥1.The WB results fed back by Center for Disease Control and Prevention were used as the standard of HIV antibody de-tection,and the detection results of LiCA800 were evaluated by statistical analysis.Results The 53 samples with LiCA800 test results S/CO≥1 were screened by enzyme-linked immunosorbent assay(ELISA)and RT for preliminary screening test:24 cases were negative for both screening methods,and the remaining 29 reactivity samples were confirmed by WB antibody test,WB bands were negative in 6 cases(11.32%),WB bands were uncer-tain in 7 cases(13.21%),and WB bands were positive in 16 cases(30.19%).The S/CO values of WB positive samples ranged from 30.31-223.97,S/CO value>100 accounted for 75.00%of the cases.The S/CO value of WB positive samples was significantly higher than that of WB uncertain,WB negative and ELISA and RT negative samples,and the differences were statistically significant(P<0.05).Among the positive results of WB,the oc-currence rate of gp160,gp120 and p24 was 100.00%,and the occurrence rate of p55 was 43.75%.Conclusion The positive coincidence rate of Li-CA800 was only 30.19%,and there was a certain relationship between S/CO value and the result of HIV antibody confirmation,experimentalists should pay attention to S/CO values and WB results to avoid disputes.
