摘　　要：目的了解北京市昌平区医疗机构消毒质量状况,为改进辖区医疗机构消毒工作质量和院内感染控制提供依据。方法2018—2022年分层抽取北京市昌平区647家次医疗机构开展医疗机构消毒质量连续监测,采样及检测方法参照《消毒技术规范》、《医院消毒卫生标准》等相关标准,统计学分析用SPSS 18.0软件,合格率差异比较用χ^(2)检验,P<0.05为差异有统计学意义。结果2018—2022年北京市昌平区医疗机构共采样监测5497件,合格5466件,总合格率为99.44%,其中2020年合格率最低(98.75%),2018年和2022年最高,均为99.91%,各年度的监测合格率差异有统计学意义(χ^(2)=20.269,P<0.001);监测项目中,压力灭菌锅和污水合格率最高,均为100.00%;室内空气合格率最低,为97.63%,不同监测项目合格率差异有统计学意义(χ^(2)=38.265,P<0.001);不同级别医疗机构监测合格率差异有统计学意义(χ^(2)=9.772,P=0.017),呈现医疗机构级别越低合格率也越低趋势(χ^(2)趋势=8.704,P=0.003)。结论2018—2022年北京市昌平区医疗机构消毒质量总体合格率较高,但医护人员手和室内空气的消毒质量仍需改进,特别是应重点加强一级及以下医疗机构的消毒质量监测和指导。Objective To investigate the disinfection quality of medical institutions in Changping District of Beijing,and to provide evidence for improving the disinfection quality of medical institutions and nosocomial infection control.Methods About 647 medical institutions in Changping District of Beijing were selected for continuous monitoring.The sampling and detection methods were based on relevant standards such as Disinfection Technical Specifications and Hygienic Standards for Hospital Disinfection.SPSS 18.0 software was used for statistical analysis,and the difference of pass rate was compared by Chi-square test.The difference P<0.05 was considered statistically significant.Results A total of 5497 samples were collected and monitored in medical institution in Changping district of Beijing during 2018 to 2022,among them,5466 samples were qualified,with a total pass rate of 99.44%.The lowest pass rate was 98.75%in 2020,and the highest was99.91%in 2018 and 2022.The difference was statistically significant(χ^(2)=20.269,P<0.001).In the monitoring items,the qualified rate of pressure sterilization pot and sewage was the highest,all were 100.00%;The indoor air qualified rate was 97.63%,and there was significant difference among different monitoring items(χ^(2)=38.265,P<0.001).The difference of qualified rate of different medical institutions was statistically significant(χ^(2)=9.772,P=0.017),the lower the leve of medical institutions,the lower the pass rate(χ^(2)_(Trend)=8.704,P=0.003).Conclusions From 2018to 2022,the overall pass rate of disinfection quality in medical institutions in Changping District of Beijing is relatively high,but the disinfection quality of medical workers′hands and indoor air still need to be improved,especially the disinfection quality monitoring and guidance of medica institutions at grade 1 and below should be strengthened.
