北京市某三甲医院骨科住院患者疾病构成分析

摘　　要：目的了解北京市某三甲医院骨科住院患者的疾病构成及分布情况,为骨科专科建设以及医疗资源合理配置提供数据支持。方法收集2019年1月1日至2021年12月31日前来医院骨科就诊患者的住院病案首页资料,回顾性分析骨科住院患者疾病构成、顺位变化趋势、年龄和性别分布。结果共收集骨科住院患者96746例,其中男性48568例,女性48178例;60岁及以上患者人数最多(n=28898例),占比29.87%。前10位病种分别为膝关节病(9.34%)、取出骨折内固定装置(7.75%)、股骨骨折(6.49%)、小腿(包括踝)骨折(6.21%)、前臂骨折(3.89%)、其他脊椎病(3.61%)、在腕和手水平的骨折(3.33%)、肩和上臂骨折(3.11%)、髋关节病(3.11%)和其他椎间盘疾患(3.09%),累计占比49.92%。膝关节病和股骨骨折在女性和≥60岁年龄组患者中均位于第一顺位和第二顺位;取出骨折内固定装置、小腿骨折以及腕关节骨折在男性和20岁-39岁年龄组患者中均位于前三顺位。结论膝关节病、取出骨折内固定装置和股骨骨折为该院的优势病种。不同性别和年龄段的骨科住院患者主要病种构成存在较大差异。Objective To understand the disease composition,changes and common diseases of orthopedic inpatients in a Three A and Tertiary Hospital in Beijing,and provide data support for the construction of orthopedics,and the rational allocation of medical resources.Methods Data from the front page of medical records of orthopedic inpatients from January 1st,2019 to December 31st,2021 were collected.Disease composition,sequence change trend,age and sex distribution of orthopedic inpatients was analyzed.Results Among hospitalized orthopedic patients from 2019 to 2021,there were 48568 males(50.20%)and 48178 females(49.80%).Patients aged 60 years and above account for largest proportion(n=28898,29.87%).The top ten diseases were arthrosis of knee(9.34%),removal of fracture internal fixation devices(7.75%),femoral fracture(6.49%),fracture of lower leg,including ankle(6.21%),fracture of forearm(3.89%),other spondylopathies(3.61%),fractures at wrist and hand level(3.33%),fractures of shoulder and upper arm(3.11%),arthrosis of hip(3.11%),and other intervertebral disc disorders(3.09%),with a cumulative percent of 49.92%.For female and patients aged above 60years,arthrosis of hip,femoral fracture both ranked the top 2,and for male and patients aged 20-39 years,the top three diseases were removal of fracture internal fixation devices,fracture of lower leg and fractures at wrist and hand level.Conclusions Arthrosis of knee,removal of fracture internal fixation devices,and femoral fracture were most common diseases among orthopedic inpatients.The disease distribution of orthopedic inpatients varied with gender and age.
