摘　　要：目的:将促进先天性心脏病婴儿术后直接母乳喂养的证据应用于临床并评价其应用效果,为护理人员开展循证实践及证据应用与临床转化提供思路。方法:遵循复旦大学循证护理中心的“证据临床转化模式”,总结促进先天性心脏病婴儿术后直接母乳喂养的相关证据,制定相应的临床审查指标,逐条审查并分析障碍因素,构建实施策略并进行实践变革。结果:与基线审查相比,护士对审查指标的执行率及证据相关知识的知晓度、直接母乳喂养维持率、纯母乳喂养及母乳喂养(包括纯母乳喂养和混合喂养)的维持率、母亲母乳喂养自信心评分得到显著提高(P<0.01)。结论:促进先天性心脏病婴儿术后直接母乳喂养的证据应用在规范护士行为,提高护士对证据相关知识的掌握度、提高先天性心脏病婴儿术后直接母乳喂养维持率、纯母乳喂养及母乳喂养的维持率、母亲母乳喂养自信心等方面存在临床意义。Objective:To apply the evidence of promoting direct breastfeeding in infants with Congenital Heart Disease(CHD)after surgery to clinical practice and evaluate its effectiveness,and to provide ideas for nursing staff to carry out evidence-based practice,evidence application,and clinical transformation.Methods:Following the"evidence clinical transformation model"of the Evidence based Nursing Center of Fudan University,we summarized the evidence on promoting direct breastfeeding of infants with CHD after surgery,formulated the corresponding clinical review indicators,reviewed and analyzed the obstacles one by one,constructed the implementation strategy and carried out practical reform.Results:Compared with the baseline review,nurses'awareness of the implementation rate and evidence related knowledge of the review indicators,maintenance rate of direct breastfeeding,maintenance rate of exclusive breastfeeding and breastfeeding(including exclusive breastfeeding and mixed feeding),and maternal breastfeeding confidence score were significantly improved(P<0.01).Conclusion:The application of evidence to promote direct breastfeeding in infants with CHD after surgery has clinical significance in regulating the behavior of nurses,improving their mastery of evidence-based knowledge,improving the maintenance rate of direct breastfeeding in infants with CHD after surgery,the maintenance rate of pure breastfeeding and breastfeeding,and maternal breastfeeding confidence.
