舒筋止眩汤联合盐酸氟桂利嗪治疗颈源性眩晕痰湿阻络证临床研究

摘　　要：目的观察舒筋止眩汤联合盐酸氟桂利嗪治疗颈源性眩晕痰湿阻络证的临床效果。方法随机将60例颈性眩晕病人分为治疗组与对照组,两组各30例;治疗组口服舒筋止眩汤联合盐酸氟桂利嗪胶囊治疗,对照组口服盐酸氟桂利嗪胶囊治疗,疗程2 w,并比较两组患者的治疗效果及治疗前后血液流变学与血流动力学的变化情况。结果治疗组的总有效率为86.67%,明显优于对照组的63.33%,差异有统计学意义(P<0.05);治疗后,两组的血液流变学指标(全血高切黏度、血浆黏度、红细胞压积、全血低切黏度)、血流动力学指标(椎-基底动脉平均血流速度)均较治疗前有所改善,且治疗组各指标均优于对照组,差异有统计学意义(P<0.05);治疗过程中,两组患者未出现明显心慌胸闷、恶心呕吐、腹痛腹泻或眩晕症状加重等不良反应。结论舒筋止眩汤联合盐酸氟桂利嗪胶囊治疗颈性眩晕更有优势,能有效降低血液黏度及改善血液循环,促进脑部供血,进而改善眩晕、上肢麻木、颈项僵硬疼痛等症状。Objective To explore the clinical effect of Shujin Zhixuan decoction combined with Flunarizine Hydrochloride in the treatment of cervical vertigo with phlegm dampness obstruction syndrome.Methods Sixty patients with cervical vertigo were randomly divided into a treatment group and a control group,with 30 cases in each group;The treatment group received oral administration of Shujin Zhixuan decoction combined with Flunarizine Hydrochloride capsules for treatment,while the control group received oral administration of Flunarizine Hydrochloride capsules for treatment for 2 weeks.The treatment effects and changes in blood rheology and hemodynamics before and after treatment were compared between the two groups of patients.Results The total effective rate of the treatment group was 86.67%,significantly better than the control group's 63.33%,with a statistically significant difference(P<0.05);After treatment,the hemorheological indicators(whole blood high shear viscosity,plasma viscosity,hematocrit,whole blood low shear viscosity)and hemodynamic indicators(vertebral basilar artery average blood flow velocity)of both groups improved compared to before treatment,and the treatment group had better indicators than the control group,with statistical significance(P<0.05);During the treatment,both groups of patients did not experience significant adverse reactions such as palpitations,chest tightness,nausea and vomiting,abdominal pain,diarrhea,or worsening symptoms of dizziness.Conclusion The combination of Shujin Zhixuan decoction and Flunarizine Hydrochloride capsules has more advantages in treating cervical vertigo.It can effectively reduce blood viscosity and improve blood circulation,promote cerebral blood supply,and thereby improve symptoms such as dizziness,upper limb numbness,and neck stiffness and pain.
