摘　　要：终末期肾脏病(end-stage renal disease, ESRD)患者的治疗具有长期性、独立性等特点,院外治疗可能出现感染、心衰、钙磷代谢紊乱等问题,对患者疾病知识水平、护理操作能力及社会家庭支持程度等要求较高,因此,患者延续性健康管理需求较高。本文对延续性健康管理需求的概念、需求内容及评估工具等方面进行综述,以期为未来开展相关干预提供参考。Objective The treatment of patients with end-stage renal disease was characterized by long-term and independence,and out-of-hospital treatment was prone to problems such as infection,heart failure,calcium and phosphorus metabolism disorders,which had higher requirements for disease knowledge,nursing operation ability,and social and family support of patients.Therefore,patients had a higher needs for transitional health management.This article reviewed the concept,importance of assessment,content and assessment tools of transitional health management needs,in order to provide reference for future interventions related to transitional health management in patients with end-stage renal disease.
