2016—2022年临沂市HIV/AIDS患者抗病毒治疗及时性及影响因素分析

摘　　要：目的:分析山东省临沂市2016—2022年新报告人类免疫缺陷病毒(HIV)/艾滋病(AIDS)患者抗病毒治疗及时性及影响因素,为临沂市抗病毒治疗管理工作提供参考依据。方法:选取临沂市2016—2022年新报告的1764例HIV/AIDS患者。通过艾滋病防治基本信息系统下载病例报告和抗病毒治疗历史卡片,关联后形成数据库;采用多因素logistic回归模型分析抗病毒治疗及时性及影响因素。结果:1764例患者的抗病毒治疗及时率为50.23%,不同年份抗病毒治疗及时率呈增长趋势。多因素logistic回归分析结果显示,年龄、人群分类、样本来源、临沂市发现报告、首次CD4细胞计数是抗病毒治疗及时性的影响因素。结论:临沂市新报告HIV/AIDS患者30 d内启动抗病毒治疗的比例还有待提高,相关部门应重点关注低年龄段人群、首次CD4计数较高人群、羁押人群、商业服务人群和外市报至本市的HIV/AIDS患者的随访和检测工作,完善信息沟通和转介流程,缩短从确诊到治疗的时间间隔,从而进一步提高临沂市HIV/AIDS患者的抗病毒治疗及时性。OBJECTIVE To analyze the timeliness and influencing factors of antiviral treatment for newly reported human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)patients in Linyi from 2016 to 2022,and provide reference for antiviral treatment management in Linyi.METHODS A total of 1764 newly reported HIV/AIDS patients from 2016 to 2022 were selected from Linyi.Download case reports and antiviral treatment history cards through the AIDS prevention and treatment basic information system,and form a database after association.Multivariate logistic regression model was used to analyze the timeliness and influencing factors of antiviral therapy.RESULTS The timely rate of antiviral treatment in 1764 HIV/AIDS patients was 50.23%,and the timely rate of antiviral treatment in different years showed an increasing trend.Multivariate logistic regression results showed that age,population classification,sample source,discovery report from Linyi,and first CD4 cell count were the influencing factors of ART timeliness.CONCLUSION In Linyi,the proportion of HIV/AIDS patients who started antiviral treatment within 30 days needs to be improved.Relevant departments should focus on the follow-up and testing of the low age group,the group with high CD4 count for the first time,the detention group,the commercial service group and the HIV/AIDS patients reported to the city from other cities,and improve the information communication and referral process.Shorten the time interval from diagnosis to treatment to further improve the timeliness of ART for HIV/AIDS patients in Linyi.
