温中定眩汤治疗痰浊中阻型梅尼埃病的临床研究

摘　　要：目的:观察温中定眩汤治疗痰浊中阻型梅尼埃病的临床效果。方法:选择2021年3月—2023年3月于常州市中医医院就诊的痰浊中阻型梅尼埃病患者82例,采用随机数字表法分成常规组与中药组,每组41例。常规组口服甲磺酸倍他司汀片治疗,中药组在此基础上给予口服温中定眩汤治疗。观察两组临床疗效、中医证候积分、眩晕、耳鸣、听力评分、椎动脉收缩期峰值流速与不良反应。结果:中药组治疗总有效率高于常规组(P<0.05);治疗前,两组中医证候积分、眩晕障碍量表(DHI)与耳鸣致残量表(THI)评分、纯音听力与椎动脉收缩期峰值流速差异均无统计学意义(P>0.05);治疗后,中药组中医证候积分、DHI与THI评分均低于常规组(P<0.05),中药组纯音听力与椎动脉收缩期峰值流速均高于常规组(P<0.01);两组不良反应总发生率差异不明显(P>0.05)。结论:温中定眩汤治疗痰浊中阻型梅尼埃病可有效改善临床症状、提高患者听力与椎动脉收缩期峰值流速,疗效明确且安全性较高。Objective:To observe the clinical effect of Wenzhong Dingxuan decoction(WDD)on treating Meniere's disease with phlegm turbidity obstruction.Methods:Eighty-two patients with phlegm turbidity obstruction syndrome Meniere's disease who were treated at Changzhou Hospital of Traditional Chinese Medicine from March 2021 to March 2023 were selected and randomly divided into a conventional group and a traditional Chinese medicine group using a random number table method,with 41 cases in each group.The conventional group was treated with oral betahistine mesylate tablets,while the traditional Chinese medicine group was treated the same plus oral WDD.The clinical efficacy,TCM syndrome score,vertigo,tinnitus,hearing score,vertebral artery systolic peak velocity and adverse reactions were observed.Results:The total effective rate of the traditional Chinese medicine group was higher than that of the conventional group(P<0.05).Before treatment,there was no significant difference in TCM syndrome score,dizziness handicap inventory(DHI),tinnitus handicap inventory(THI)scores,pure tone hearing,and peak systolic velocity of vertebral artery between the two groups(P>0.05).After treatment,the traditional Chinese medicine syndrome score,DHI,and THI scores in the traditional Chinese medicine group were lower than those in the conventional group(P<0.05),while the pure tone hearing and peak systolic flow velocity of the vertebral artery in the traditional Chinese medicine group were higher than those in the conventional group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Wenzhong Dingxuan decoction can effectively improve clinical symptoms,improve hearing and vertebral artery systolic peak flow rate in the treatment of phlegm turbidity obstruction syndrome Meniere's disease,with clear curative effect and high safety.
