七氟烷和丙泊酚全麻复合硬膜外阻滞麻醉对宫颈癌患者术后的影响

摘　　要：目的:探究七氟烷和丙泊酚全麻复合硬膜外阻滞麻醉对宫颈癌患者术后的影响。方法:选取2022年3月—2023年3月菏泽市中医医院收治的宫颈癌患者70例为研究对象,依据随机排列表法分为研究组和对照组,各35例。对照组应用七氟烷和丙泊酚全麻,研究组在对照组基础上应用硬膜外阻滞麻醉。比较两组患者苏醒情况、免疫指标、麻醉质量、麻醉不良反应。结果:研究组自主呼吸恢复时间、清醒时间、胃肠恢复时间短于对照组,差异有统计学意义(P=0.001);术后12、24 h,两组患者CD3^(+)、CD4^(+)/CD8^(+)水平均低于麻醉前,研究组高于对照组,差异有统计学意义(P<0.05);研究组麻醉优良率高于对照组,差异有统计学意义(P=0.046);两组麻醉不良反应发生率比较,差异无统计学意义(P>0.05)。结论:应用七氟烷和丙泊酚全麻复合硬膜外阻滞麻醉,能够使患者尽快苏醒,提高患者免疫功能及麻醉优良率,且安全性较高。Objective:To explore the effect of sevoflurane and propofol combined with epidural block anesthesia on patients with cervical cancer after surgery.Methods:Seventy cervical cancer patients treated in Heze Hospital of Traditional Chinese Medicine from March 2022 to March 2023 were selected as the research objects,and were divided into study group and control group according to random ranking table method,with thirty-five cases in each group.The control group received sevoflurane and propofol general anesthesia,and the study group received epidural block anesthesia based on the control group.The recovery,immune indexes,anesthesia quality and anesthesia adverse reactions were compared between the two groups.Results:The recovery time of spontaneous respiration,waking time and gastrointestinal recovery time in the study group were shorter than those in the control group,and the difference was statistically significant(P=0.001);at 12 and 24 h after surgery,the levels of CD3^(+)and CD4^(+)/CD8^(+)in 2 groups were lower than before anesthesia,and those in the study group were higher than those in the control group,the difference was statistically significant(P<0.05);the rate of excellent and good anesthesia in the study group was higher than that in the control group,the difference was statistically significant(P=0.046);there was no significant difference in the incidence of anesthesia adverse reactions between the two groups(P>0.05).Conclusion:Applying sevoflurane and propofol general anesthesia combined with epidural block anesthesia can revive patients as soon as possible,improve the immune function of patients and the rate of anesthesia excellence and good,and have high safety.
