硕通镜碎石术与输尿管镜气压弹道碎石术治疗上尿路结石合并脓肾患者的效果

摘　　要：目的:探讨硕通镜碎石术与输尿管镜气压弹道碎石术治疗上尿路结石合并脓肾患者的效果。方法:选择2021年4月—2023年4月枝江市人民医院收治的85例上尿路结石合并脓肾患者作为研究对象,根据手术方式不同分为观察组(硕通镜碎石术,n=42)和对照组(输尿管镜气压弹道碎石术,n=43)。比较两组手术及住院指标、结石清除情况、肾功能及并发症发生情况。结果:观察组手术时间、住院时间较对照组短,且术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组术后1 d、1个月结石清除率较对照组高,差异有统计学意义(P<0.05)。术前1 d,两组肾功能指标比较,差异无统计学意义(P>0.05);出院时,两组肾功能指标均改变,且观察组α_(1)微球蛋白(α_(1)-MG)、血尿素氮(BUN)、血肌酐(Scr)、血清胱抑素C(Cys C)水平较对照组低,差异有统计学意义(P<0.05)。观察组并发症发生率(9.52%)低于对照组(27.91%),差异有统计学意义(P<0.05)。结论:硕通镜碎石术治疗上尿路结石合并脓肾患者不仅能缩短手术及住院时间,减少术中出血量,提高结石清除率,同时也能改善患者肾功能,减少并发症的发生。Objective:To explore the effect of Shuo Tong Mirror lithotripsy and ureteroscopic pneumatic ballistic lithotripsy in the treatment of patients with upper urinary tract stones and pyonephritis.Method:A total of 85 patients with upper urinary tract stones and pyonephritis who admitted to Zhijiang People's Hospital from April 2021 to April 2023 were selected as the research objects,they were divided into the observation group(Shuo Tong Mirror lithotripsy,n=42)and the control group(ureteroscopic pneumatic ballistic lithotripsy,n=43)according to different surgical methods.The operation and hospitalization indexes,stone clearance,renal function and complications were compared between two groups.Result:The operation time and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(P<0.05).The stone clearance rate in the observation group at 1 d and 1 month after surgery were higher than those of the control group,and the differences were statistically significant(P<0.05).At 1 d before operation,there were no significant differences in renal function indexes between two groups(P>0.05);at discharge,the renal function indexes of two groups changed,and the levels ofα_(1)-microglobulin(α_(1)-MG),blood urea nitrogen(BUN),serum creatinine(Scr)and serum cystatin C(Cys C)in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group(9.52%)was lower than that in the control group(27.91%),the difference was statistically significant(P<0.05).Conclusion:The safety of Shuo Tong Mirror lithotripsy in the treatment of upper urinary tract stones and pyonephritis can not only shorten the operation and hospitalization time,reduce the intraoperative blood loss,improve the stone clearance rate,but also improve the renal function of patients and reduce the occurrence of com
