浅表高频超声联合经直肠腔内超声诊断肛周脓肿的价值

摘　　要：目的:探究浅表高频超声联合经直肠腔内超声诊断肛周脓肿的价值。方法:选取2020年2月—2022年2月绵阳市中心医院收治的疑似肛周脓肿患者136例作为研究对象,所有患者术前接受浅表高频超声检查与经直肠腔内超声扫描,以临床综合检查结果为“金标准”,比较浅表高频超声检查与联合检查的诊断效能。结果:临床综合检查显示,86例患者存在肛周脓肿。联合检查诊断肛周脓肿的准确度、灵敏度、特异度高于浅表高频超声检查,差异有统计学意义(P<0.05)。联合检查病灶总数、病灶相通数、多发病灶检出率高于浅表高频超声检查,差异有统计学意义(P<0.05)。联合检查对肛周皮下脓肿、坐骨直肠窝脓肿、直肠黏膜下脓肿、骨盆直肠窝脓肿、直肠后脓肿的检出率高于浅表高频超声检查,差异有统计学意义(P<0.05)。结论:浅表高频超声联合经直肠腔内超声诊断肛周脓肿的价值较高,病灶检出率及不同病灶类型检出率高。Objective:To study the value of superficial high-frequency ultrasound combined with transrectal ultrasound in the diagnosis of perianal abscess.Methods:A total of 136 patients suspected with perianal abscess admitted to Mianyang Central Hospital from February 2020 to February 2022 were selected as the study subjects.All patients received superficial high frequency ultrasound and transrectal ultrasound before surgery.The results of comprehensive examination were used as“gold standard”.The diagnostic efficiency of superficial high-frequency ultrasound and combined examination was compared.Results:The comprehensive examination showed that,86 cases had perianal abscess.The accuracy,sensitivity and specificity of combined examination in diagnosing perianal abscess were higher than those of superficial high-frequency ultrasound,and the difference was statistically significant(P<0.05).The total number of lesions,the number of same lesions and the detection rate of multiple lesions by combined examination were higher than those by superficial high-frequency ultrasound,and the difference was statistically significant(P<0.05).The detection rate of perianal subcutaneous abscess,ischiorectal abscess,rectal submucosal abscess,pelvirectal abscess and retrorectal abscess by combined ultrasound was higher than that by superficial high-frequency ultrasound,and the difference was statistically significant(P<0.05).Conclusion:The combination of superficial high-frequency ultrasound and transrectal ultrasound has a high value for the diagnosis of perianal abscess,and the detection rate of lesions and different lesion types is high.
