针灸联合补阳还五汤治疗脊髓损伤后神经源性膀胱临床观察

摘　　要：目的:观察针灸联合补阳还五汤治疗脊髓损伤后神经源性膀胱的临床疗效。方法:选取金溪县中医院2019年12月至2022年5月诊治的脊髓损伤后神经源性膀胱患者78例,按照随机数字表法分为对照组和观察组,每组39例。对照组给予常规康复治疗,观察组给予针灸联合补阳还五汤治疗。观察两组患者治疗前后尿流动力学指标、排尿情况、膀胱功能积分、生活质量评分变化情况及临床疗效。结果:观察组有效率为94.87%,对照组有效率为79.49%,观察组有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后躯体功能、社会功能、物质生活状态及心理功能评分高于对照组,差异有统计学意义(P<0.05);观察组治疗后尿流动力学指标及排尿情况均优于对照组,差异有统计学意义(P<0.05);观察组治疗后膀胱功能积分低于对照组,差异有统计学意义(P<0.05)。结论:针灸联合补阳还五汤治疗脊髓损伤后神经源性膀胱,可增强患者膀胱功能,改善尿流动力学和排尿情况,提高患者生活质量。Objective:To observe the clinical efficacy of acupuncture combined with Yang⁃Supplementing and Five⁃Returning Decoction on neurogenic bladder after spinal cord injury.Methods:A total of 78 patients with neurogenic bladder after spinal cord injury in Jinxi County Hospital of Traditional Chinese Medicine from December 2019 to May 2022 were selected and divided into the control group and the observation group according to the random number table method,with 39 cases in each group.The control group was given conven⁃tional rehabilitation treatment,while the observation group was given acupuncture combined with Yang⁃Supplementing and Five⁃Retur⁃ning Decoction.The changes in urodynamic indexes,micturition,bladder function score,quality of life score and clinical efficacy of the two groups before and after treatment were observed.Results:The effective rate of the observation group was 94.87%,while that of the control group was 79.49%.The effective rate of the observation group was higher than that of the control group,with statistically signifi⁃cant difference(P<0.05).After treatment,the scores of physical function,social function,material life status and psychological func⁃tion in the observation group were higher than those in the control group,with statistically significant differences(P<0.05);The uro⁃dynamic indexes and micturition in the observation group were better than those in the control group,with statistically significant differ⁃ences(P<0.05);The bladder function score in the observation group was lower than that in the control group after treatment,with sta⁃tistically significant differences(P<0.05).Conclusion:Acupuncture combined with Yang⁃Supplementing and Five⁃Returning Decoc⁃tion for the treatment of neurogenic bladder after spinal cord injury can enhance the bladder function,improve urodynamics and micturition,and improve the quality of life of patients.
