除痹汤结合针灸治疗类风湿关节炎的效果

摘　　要：目的:探究除痹汤结合针灸治疗类风湿关节炎(RA)的临床效果及对风湿因子、炎症情况的影响。方法:选择2022年1—12月在恩施市中心医院就医的88例RA患者,随机分为对照和联合组,各44例。对照组予以针灸和常规西医治疗,联合组在对照组的基础上结合除痹汤治疗。观察两组入院时及治疗3个疗程后的主要症状情况、风湿因子水平及炎症情况。结果:治疗3个疗程后,两组晨僵时间明显短于入院时,关节肿胀评分和关节视觉模拟评分法(VAS)评分明显低于入院时,且联合组晨僵时间明显短于对照组,关节肿胀评分和关节VAS评分明显低于对照组,差异有统计学意义(P<0.05)。治疗3个疗程后,两组风湿因子指标均低于入院时,且联合组免疫球蛋白G(IgG)、外周血红细胞沉降率(ESR)及血清类风湿因子(RF)水平明显低于对照组,差异有统计学意义(P<0.05)。治疗3个疗程后,两组炎症指标均明显低于入院时,且联合组血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及超敏C反应蛋白(hs-CRP)均低于对照组,差异有统计学意义(P<0.05)。结论:除痹汤结合针灸可明显改善RA患者的主要临床症状,降低患者的风湿因子水平和炎症反应程度。Objective:To explore the clinical effect of Chubi Decoction combined with acupuncture on rheumatoid arthritis(RA)and its influence on rheumatic factors and inflammation.Method:A total of 88 RA patients treated in Enshi Central Hospital from January to December 2022 were randomly divided into control group and combination group,with 44 patients in each group.The control group was treated with acupuncture and conventional western medicine,and the combination group was treated with Chubi Decoction on the basis of the control group.The main symptoms,levels of rheumatic factors and inflammation were observed at admission and after 3 courses of treatment.Result:After 3 courses of treatment,the morning stiffness time of the two groups was significantly shorter than that at admission,the joint swelling score and joint visual analogue scale(VAS)score were significantly lower than those at admission,and the morning stiffness time of the combined group was significantly shorter than that of the control group,and the joint swelling score and joint VAS score were significantly lower than those of the control group,the differences were statistically significant(P<0.05).After 3 courses of treatment,rheumatoid factor indexes in both groups were lower than those at admission,and immunoglobulin G(IgG),peripheral blood erythrocyte sedimentation rate(ESR)and serum rheumatoid factor(RF)levels in the combination group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After 3 courses of treatment,the inflammatory indexes in both groups were significantly lower than those at admission,and the serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and hypersensitive C-reactive protein(hs-CRP)in the combined group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Chubi Decoction combined with acupuncture can significantly improve the main clinical symptoms of RA patients,reduce the level of rheumatic factors and inf
