摘　　要：利用功能磁共振观察颞叶癫痫伴执行功能障碍患者丘脑—背外侧前额叶通路功能连接(functional connectivity,FC)特征。方法回顾性分析32例颞叶癫痫患者,对比颞叶癫痫执行功能正常者、执行功能损害者与健康对照组三组间丘脑—背外侧前额叶间FC及海马体积的差异,并观察这些存在显著差异的参数与威斯康星卡片分类测试(wisconsin Card Sorting Test,WCST)的相关性。结果颞叶癫痫伴执行功能障碍患者表现为致痫灶侧丘脑—背外侧前额叶间FC破坏,且与WCST评分呈显著负相关。海马体积与WCST评分无显著相关性。结论丘脑—背外侧前额叶连接通路或许是与衡量颞叶癫痫患者执行功能的一个重要特征。Objective To observe the functional connectivity(FC)of thalamus-dorsolateral prefrontal pathway in patients with temporal lobe epilepsy showing executive dysfunction by using functional magnetic resonance imaging.Methods Thirty-two patients with temporal lobe epilepsy were analyzed retrospectively,and the differences of FC and hippocampus volume between thalamus and dorsolateral prefrontal cortex among patients with normal executive function,patients with executive dysfunction and healthy controls were compared,and the correlation between these parameters with significant differences and Wisconsin Card Sorting Test(WCST)was observed.Results The temporal lobe epilepsy patients with executive control dysfunction showed decreased FC between thalamus and dorsolateral prefrontal lobe pathway,which was significantly related to executive function performance.There is no significant correlation between hippocampal volume and executive function.Conclusion The dorsal lateral prefrontal-thalamic connection pathway may be an important feature to measure the executive function of patients with temporal lobe epilepsy.
