摘  要:目的 探讨糖化血红蛋白(HbA1c)测定在糖尿病诊断中的临床应用价值。方法 选取本院2020年1—12月接诊的糖尿病患者作为研究对象,共900例,作为观察组;另在同期体检健康者中选取900例,作为对照组。所有受检者均接受血糖、HbA1c的测定,对比观察两组受检者血糖指标、HbA1c测定的变化;同时计算分别以空腹血糖(FBG)、餐后2h血糖(2hPBG)、HbA1c检测糖尿病的灵敏度、特异度。结果 观察组HbA1c、FBG、2hPBG指标水平均高于对照组,差异有统计学意义(P<0.05)。观察组首次FBG、2hPBG指标水平较二次检测水平存在统计学意义(P<0.05);HbA1c指标水平较二次检测差异无统计学意义(P>0.05)。FBG在糖尿病诊断中的灵敏度、特异度分别为98.00%、80.56%;2hPBG在糖尿病诊断中的灵敏度、特异度分别为97.67%、89.89%;HbA1c在糖尿病诊断中的灵敏度、特异度分别为98.44%、97.67%。结论 糖尿病患者HbA1c、FBG、2hPBG指标水平均显著高于健康人,但FBG、2hPBG在不同时间检测差异较大,而HbA1c相对更为稳定,以HbA1c进行糖尿病的诊断可靠性更高,能获得更高的特异度,显著降低漏诊的发生。Objective To explore clinical value of glycosylated hemoglobin(HbA1c) in diagnosis of diabetes.Methods The paper chose 900 patients with diabetes from January 2020 to December 2020 in our hospital as study subjects in observation group,and 900 healthy individuals for physical examination during the same period as control group.All subjects were measured with blood glucose and HbA1c,changes in blood glucose and HbA1c measurements were compared and observed between two groups.At the same time,sensitivity and specificity of fasting blood glucose(FBG),2h postprandial blood glucose(2hPBG) and HbA1c was calculated to detect diabetes.Results Levels of HbA1c,FBG,and 2-hour PBG indicators in observation group were higher than control group,difference was statistically significant(P<0.05).The first FBG and 2-hour PBG index levels in observation group were statistically significant compared with the second detection level(P<0.05).There was no statistically significant difference in HbA1c index level compared with the secondary detection,(P>0.05).Sensitivity and specificity of FBG in diagnosis of diabetes was 98.00% and 80.56% respectively.Sensitivity and specificity of 2hPBG in diagnosis of diabetes was 97.67% and 89.89% respectively.Sensitivity and specificity of HbA1c in diagnosis of diabetes was 98.44% and 97.67% respectively.Conclusion Levels of HbA1c,FBG and 2hPBG of diabetes patients are significantly higher than healthy people,detection of FBG and 2hPBG at different times varies greatly,and HbA1c is relatively more stable.Diagnosis of diabetes with HbA1c is reliable,can obtain high specificity,and reduce incidence of missed diagnosis significantly.
