摘 要:目的 查明滕州市首例人感染猪链球菌病例的传染源和传播途径,为预防控制疫情提供依据。方法 通过询问病史和流行病学史、临床表现、血培养分离猪链球菌等实验室检测进行调查。结果 患者女性,48岁,既往脾切除病史。2022年7月14日急性起病,全身感染症状和神经系统中毒症状,7月18日检验报告显示药敏结果为猪链球菌感染,患者无明确死猪接触史,发病前有买猪肉吃猪肉史,患者临床症状符合中毒休克综合征(休克、肝肾损伤、凝血功能障碍),结合血培养结果,考虑符合人感染猪链球菌病。7月21日省疾控中心复检,鉴定结果为猪链球菌2型阳性。患者因多器官功能衰竭于7月24日抢救无效死亡。结论 患者实验室确诊为人感染猪链球菌病(猪链球菌2型),病菌可能是经破损皮肤伤口感染。做好生猪疫情监测,加强管理和个人防护是预防控制人感染猪链球菌病的主要措施。Objective To provide a basis for preventing and controlling the epidemic by identifying the source and transmission route of the first human case infection with Streptococcus suis in Tengzhou City.Methods Con-duct investigations by inquiring about medical and epidemiological history,clinical manifestations,and conducting laboratory tests such as isolating Streptococcus suis through blood culture.Results The patient is a 48-year-old female with a history of splenectomy,and acute onset on July 14,2022,with symptoms of systemic infection and neurological toxicity.On July 18,the test report showed that the drug sensitivity test was Streptococcus suis.The pa-tient had no clear history of contact with dead pigs and had a history of buying and eating pork before the onset of the disease.The patient's clinical symptoms were in line with toxic shock syndrome(shock,liver and kidney dam-age,coagulation dysfunction).Combined with blood culture results,it was considered that she were in line with hu-man infection with Streptococcus suis.On July 21,the Provincial Center for Disease Control and Prevention con-ducted a retest and identified positive results for Streptococcus suis type 2.The patient died on July 24 due to mul-tiple organ failure and failed rescue efforts.Conclusion The patient confirmed human infection with Streptococ-cus suis by laboratory examination(Streptococcus suis type 2),and the pathogen may have been infected through damaged skin wounds.Monitoring the epidemic situation in live pigs,strengthening management and personal pro-tection are the main measures to prevent and control human infection with Streptococcus suis.
