“以补助通法”治疗PCI术后胸痛的理论探讨

摘　　要：患者在经皮冠状动脉介入术(PCI)后,经常会出现胸痛、胸闷、心悸等症状。对于PCI术后胸痛的治疗,中医学有着得天独厚的优势。冠心病PCI术后胸痛的基本病机本虚标实,病位主要在心,气虚血瘀、气虚痰瘀、阳虚寒凝、阴虚毒蕴为冠心病PCI术后胸痛的病机关键。本文在此基础上提出“以补助通法”并衍生出益心气通血瘀法、益心气通痰瘀法、温心阳通阴寒法、滋心阴通热毒法以治疗此病,为临床治疗冠心病PCI术后胸痛提供新的诊疗思路。Patients may experience symptoms of chest pain,chest tightness,and palpitations after percutaneous coronary intervention(PCI).Traditional Chinese medicine has unique advantage in treatment of chest pain after PCI.Basic pathogenesis of post PCI chest pain of coronary heart disease is deficiency in origin and excess in superficislity,disease mainly located in the heart,key pathogenesis of post PCI chest pain was Qi deficiency and blood stasis,Qi deficiency and phlegm stasis,YANG deficiency and cold coagulation,and YIN deficiency and toxin accumulation.The author proposed"method of promoting blood circulation by supplementing"and derived methods of tonifying heart qi and promoting blood stasis,tonifying heart qi and promoting phlegm stasis,warming heart YANG and promoting YIN and cold,nourishing heart YIN and promoting heat and toxin to treat the disease,providing new diagnostic and therapeutic ideas for clinical treatment of post PCI chest pain in coronary heart disease on the basis.
