多药耐药铜绿假单胞菌院内感染危险因素及预后因素分析

摘　　要：探究多药耐药铜绿假单胞菌(MDRP)院内感染危险因素、预后影响因素。选择平原县第一人民医院2021年5月—2022年5月收治的60例MDRP院内感染患者为研究对象,并随机分为对比组和分析组,每组患者各30例。对两组患者采用单因素、多因素Logistic回归进行分析。结果显示,单因素分析发现患者年龄、ICU/RCU内、支气管炎、恶性肿瘤、APACHEⅡ评分、混合感染、机械通气、分离该菌前15天服用过氟喹诺酮、亚胺培南属于危险因素;多因素Logistic回归分析确定机械通气、分离该菌前15天服用过亚胺培南属于独立危险因素。单因素分析发现APACHEⅡ评分、机械通气、MDRP未恢复敏感性为死亡危险因素;多因素Logistic回归分析发现MDRP未恢复敏感性为死亡独立危险因素(P<0.05)。研究发现,MDRP感染危险因素是机械通气、应用亚胺培南,影响患者预后的危险因素为MDRP未恢复敏感性。Exploring the risk factors and prognostic factors of hospital acquired infections caused by multidrug-resistant Pseudomonas aeruginosa(MDRP).Sixty MDRP patients admitted to the First People’s Hospital of Pingyuan County from May 2021 to May 2022 were selected as the study subjects,and randomly divided into a control group and an analysis group,with 30 patients in each group.Perform univariate and multivariate logistic regression analysis on two groups of patients.The results showed that univariate analysis revealed that patient age,ICU/RCU,bronchitis,malignant tumors,APACHE II score,mixed infection,mechanical ventilation,and the use of fluoroquinolone and imipenem 15 days prior to bacterial isolation were risk factors;Multivariate logistic regression analysis determined that mechanical ventilation and taking imipenem 15 days prior to bacterial isolation were independent risk factors.Univariate analysis found that APACHE II score,mechanical ventilation,and MDRP sensitivity not restored were risk factors for mortality;Multivariate logistic regression analysis found that MDRP did not restore sensitivity as an independent risk factor for death(P<0.05).Research has found that the risk factors for MDRP infection are mechanical ventilation and the use of imipenem,while the risk factor affecting patient prognosis is that MDRP sensitivity has not been restored.
