由Fonsecaea monophora所致以假水疱为主要表现的着色芽生菌病1例

摘　　要：报告以假水疱为主要表现的着色芽生菌病1例。患者女性,77岁,左前臂红色斑块、结痂4年,其上见细小假水疱样皮损。组织病理见混合性炎细胞浸润及硬壳细胞。组织沙堡弱培养基28℃培养见棕黑色菌落,中央呈灰黑色,菌落生长慢。小培养见枝孢和喙枝孢型混合产孢。分子测序比对为F.monophora。给予口服伊曲康唑200 mg,1次/日,盐酸特比萘芬片250 mg,1次/日,4个月后改为口服伊曲康唑200 mg,1次/日,3个月,配合温热疗法,皮损基本痊愈,随访无复发。A case of chromoblastomycosis with pseudoblister as the main manifestation is reported.The patient was a 77 year old female with red plaque and scab on her left forearm for 4 years.Histopathology showed mixed inflammatory cell infiltration and muriform cells.Brown black colonies could be seen in the tissue Sabaouraud’s medium at 28℃,the center was gray black,and the growth of colonies was slow.Cladospora and Rhinocladiella were mixed in small culture.The molecular sequencing alignment was F.monophora.Oral itraconazole 200 mg/d and terbinafine hydrochloride tablets 250 mg/d was given for 4 months,then changed to oral itraconazole 200 mg/d for 3 months.Combined with hyperthermia therapy,the skin lesions were basically cured,and there was no recurrence during follow-up.
