激光边缘切除法在小阴唇肥大整形中的临床应用

摘　　要：目的探讨激光边缘切除法行小阴唇肥大整形的临床效果。方法2022年1月~2023年10月,选择18例双侧小阴唇肥大患者,年龄18~42岁,(28.5±6.6)岁,均有外阴部小阴唇摩擦不适及运动后疼痛不适,12例有性生活史者性生活无影响。查体双侧小阴唇水平组织冗余肥大,边缘粗糙肥厚,色素沉着浓密,站立时双侧小阴唇超出大阴唇水平,测量左侧小阴唇宽度25~40 mm,(31.7±5.4)mm,右侧25~40 mm,(31.4±5.0)mm。采用边缘切除法行双侧小阴唇缩小整形,遵行双侧对称原则设计切口并画线,冷刀切开皮肤黏膜,CO 2激光切开皮下组织并严密止血,6-0可吸收缝线严密缝合。结果手术时间68~120 min,(81.5±16.2)min,切口一期愈合,无手术并发症。术后随访1~6个月,(4.3±1.4)月,摩擦、疼痛症状均消失,12例有性生活史者术后性生活无影响。术后1个月检查,双侧小阴唇形态、颜色对称,形态自然,边缘组织柔软,颜色变浅,站立时均未超出大阴唇水平,测量左侧小阴唇宽度10~15 mm,(13.7±1.2)mm,右侧10~15 mm,(13.5±1.4)mm。结论边缘切除法小阴唇缩小整形后,患者临床症状消失,小阴唇缩小效果确切,外形自然,双侧同时手术后双侧形态对称,对于小阴唇水平组织冗余肥大、边缘粗糙肥厚、色素沉着浓密者,是一种可行的小阴唇整形方法。Objective To investigate the clinical effect of laser marginal excision in labia minora hypertrophy surgery.Methods From January 2022 to October 2023,18 patients with bilateral labia minora hypertrophy,aged 18-42 years old(average,28.5±6.6 years old),were enrolled in this study.All the patients experienced discomfort from friction of the labia minora in the external genitalia and pain and discomfort after exercise,while 12 patients with a history of sexual activity had no impact on sexual activity.Examination findings showed bilateral labia minora horizontal tissue redundancy hypertrophy,with rough and hypertrophic edges and dense pigmentation.The bilateral labia minora were beyond the level of labia majora when standing,measuring left width of 25-40 mm(average,31.7±5.4 mm)and right width of 25-40 mm(average,31.4±5.0 mm).Bilateral labia minora reduction and plastic surgery was performed by marginal excision.The incorporation was designed and lines were drawn in accordance with the principle of bilateral symmetry.The skin and mucosa were cut with a cold knife,the subcutaneous tissue was incised with CO 2 laser with bleeding thoroughly stopped,and the 6-0 absorbable thread was used for tight suture.Results The operation time was 68-120 min(average,81.5±16.2 min),and the incision was healed without surgical complications.After 1-6 months of postoperative follow-ups(average,4.3±1.4 months),the symptoms of friction pain disappeared,and there was no effect on the postoperative sexual life in 12 patients with sexual history.At 1 month after surgery,the shape and color of the bilateral labia minora were symmetrical,with natural shape,soft edge tissue,and lighter color.None exceed the level of the labia majora when standing.The width of the left labia minora was measured to be 10-15 mm(average,13.7±1.2 mm),and the right labia minora was measured to be 10-15 mm(average,13.5±1.4 mm).Conclusions After marginal resection for labia minora reduction and plastic surgery,clinical symptoms of the patient disappear.The reduc
