后路椎弓根螺钉内固定联合伤椎椎体成形术对脊柱骨折患者脊柱功能、疼痛和创伤应激的影响

摘　　要：目的 探讨脊柱骨折采用后路椎弓根螺钉内固定联合伤椎椎体成形术治疗后的脊柱功能、疼痛和创伤应激指标的变化。方法 回顾性选取于2021年4月—2023年1月福州市第二总医院脊柱外科收治的82例脊柱骨折患者的临床资料,按照治疗方案分为研究组(35例)和对照组(47例),研究组采用椎弓根螺钉内固定联合伤椎椎体成形术,对照组采用单纯后路椎弓根螺钉内固定,对比两组临床疗效、脊柱功能、创伤应激水平、脊柱状态改善情况。结果 研究组治疗优良率(88.57%)高于对照组(61.70%),差异有统计学意义(χ^(2)=4.021,P<0.05);术后3 d,研究组脊柱功能评分优于对照组,创伤应激指标水平低于对照组,差异有统计学意义(P均<0.05);术后2个月,研究组脊柱状态改善情况优于对照组,差异有统计学意义(P<0.05)。结论 对于脊柱骨折患者,后路椎弓根内固定联合伤椎椎体成形手术治疗,能够促进患者脊柱功能恢复,缓解创伤应激和疼痛程度,改善预后,实用性和安全性更高。Objective To discuss the changes of spinal function,pain and traumatic stress indexes after posterior pedicle screw fixation combined with vertebroplasty of injured vertebrae.Methods Clinical data of 82 patients with spinal fracture admitted to the Spinal Surgery Department of Fuzhou Second General Hospital from April 2021 to January 2023 were retrospectively selected.According to the treatment plan,they were divided into study group(35 cases)and control group(47 cases).The study group received pedicle screw internal fixation combined with vertebra plasty of injured vertebra,and the control group received simple posterior pedicle screw internal fixation.The clinical efficacy,spinal function,traumatic stress level and improvement of spinal status were compared between the two groups.Results The excellent rate of treatment of the study group(88.57%)was higher than that of the control group(61.70%),and the difference was statistically significant(χ^(2)=4.021,P<0.05).3 days after surgery,the spinal function score of the study group was better than that of the control group,and the level of traumatic stress index was lower than that of the control group,and the differences were statistically significant(both P<0.05).Two months after operation,the spinal status in the study group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For spinal fractures patients,posterior pedicle fixation combined with vertebroplasty can promote the recovery of spinal function in patients with injured vertebroplasty,relieve traumatic stress and pain,improve prognosis,and have higher practicability and safety.
