宣肺祛痰化瘀法联合多索茶碱治疗慢性阻塞性肺疾病的疗效及对血气分析PaO_(2),PaCO_(2)的影响

摘　　要：目的:探究布宣肺祛痰化瘀法联合多索茶碱治疗慢性阻塞性肺疾病(COPD)的疗效及对血气分析血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))的影响。方法:选取2021年3月~2022年3月我院老年病科收治的120例COPD患者为研究对象,按照随机数字表法分为研究组(n=60)和对照组(n=60),对照组予多索茶碱治疗,研究组患者在此基础上加用刘方柏教授协定方宣肺祛痰方,比较两组治疗后临床有效率、中医证候积分、血气指标[pH值、PaO_(2)、PaCO_(2)]、肺功能[第一秒末用力呼气量(FEV1)、呼气流量峰值(PEF)及气道阻力(Reff值)]及生活质量[圣·乔治呼吸问卷(SGRD)]。结果:研究组临床有效率96.67%高于对照组83.33%(P<0.05);治疗后,两组中医证候积分均有所下降,且研究组低于对照组(P<0.05);治疗后,两组pH值、PaO_(2)均有所升高,且研究组高于对照组(P<0.05),PaCO_(2)均有所下降,且研究组低于对照组(P<0.05);治疗后,两组FEV1及PEF均有所升高,且研究组高于对照组,Reff均有所降低,且研究组低于对照组(P<0.05);两组SGRD评分各指标均有所降低,且研究组低于对照组(P<0.05)。结论:宣肺祛痰化瘀法联合多索茶碱治疗COPD患者疗效显著,能够有效缓解患者临床症状,增强患者肺部功能,改善血气状况,提高生活质量。Objective To explore the curative effect of Xuanfei Qutan Huayu Decoction combined with doxofylline and its influences on blood gas analysis indexes[partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2))]in chronic obstructive pulmonary disease(COPD).Methods A total of 120patients with COPD admitted to Geriatrics Department of the hospital were enrolled as the research objects between March 2021and March 2022.According to random number table method,they were divided into study group(n=60)and control group(n=60).The control group was treated with doxofylline,while study group was additionally treated with Professor Liu Fangbai concerted prescription(Xuanfei Qutan Huayu Decoction).The clinical response rate,scores of TCM symptoms,blood gas indexes(pH,PaO_(2),PaCO_(2)),lung function indexes[forced expiratory volume in 1second(FEV1),peak expiratory flow(PEF),airway resistance(Reff)]and quality of life[St.George Respiratory Questionnaire(SGRD)]were compared between the two groups after treatment.Results The clinical response rate of study group was higher than that of control group(96.67%vs 83.33%,P<0.05).After treatment,scores of TCM symptoms in both groups were decreased,which were lower in study group than control group(P<0.05).After treatment,pH and PaO_(2)were increased in both groups,which were higher in study group than control group(P<0.05),while PaCO_(2)was decreased,which was lower in study group than control group(P<0.05).After treatment,FEV1 and PEF were increased in both groups,which were higher in study group than control group(P<0.05),while Reff was decreased,which was lower in study group than control group(P<0.05).After treatment,SGRD scores were decreased in both groups,which were lower in study group than control group(P<0.05).Conclusion Xuanfei Qutan Huayu Decoction combined with doxofylline has significant curative effect in COPD patients,which can effectively relieve clinical symptoms,enhance lung function,improve blood gas status and quality of life.
