基于郑钦安“阴阳为纲,阳主阴从”思想探讨补坎益离丹治疗心力衰竭

摘　　要：心力衰竭是多种心血管疾病发展的终末阶段,以心室充盈或射血功能受损,心排血量不能满足机体组织代谢需要为特征,其病势凶险,是导致患者死亡的主要原因。郑钦安精研经典,参透人体阴阳合和之道,深得仲景立法垂方之义,其主要思想为“阴阳为纲,阳主阴从”,临证时擅长阴阳辨证与六经辨证相结合。本文将郑钦安的学术思想应用于心衰的治疗,认为心衰多以心阳衰败、阳不制阴为基本病机,六经兼证为病情加重的重要因素,治疗以“大补元阳,以壮君火”为基本原则,补坎中之真阳,处方以补坎益离丹为基本方,再据六经辨证,针对六经辨证灵活加减,以期为临床辨治心衰提供新的路径和方法。Heart failure is the final stage of the development of a variety of cardiovascular diseases.It is characterized by impaired ventricular filling or ejection function,and the failure of cardiac bleeding volume to meet the metabolic needs of body tissues,which is dangerous and is the main cause of death.ZHENG Qin'an studies the classics,understands the harmonization of yin and yang in the human body,and deeply gains the meaning of ZHANG Zhongjing's treatment methods and prescriptions.ZHENG's main ideas are based on"yin and yang being the principle,yang dominating and yin subordinating",and he is good at combining yin and yang syndrome differentiation with six channels syndrome differentiation.In this paper,the authors discuss the treatment of heart failure from ZHENG Qin'an's academic insights.It is believed that the basic pathogenesis of heart failure is the decline of heart yang and the failure of yang to control yin.The important factors that worsen the condition are complicated syndromes of six channels."Powerful supplementation of original qi and yang to strengthen heart fire"is the basic principle of treatment so as to supplement the true yang in the kidney.Bukan Yili Pellets is the basic prescription to treat the disease,and is flexibly modified according to the six channels syndrome differentiation.This paper aims to provide a new approach and method for clinical differentiation and treatment of heart failure.
