温针灸联合温通膏外敷治疗寒湿痹阻型肩周炎临床研究

摘　　要：目的:观察温针灸联合温通膏外敷治疗寒湿痹阻型肩周炎的临床疗效。方法:采用随机数字表法将98例寒湿痹阻型肩周炎患者分为治疗组、对照组各49例。对照组采用温通膏外敷治疗,治疗组在对照组基础上加用温针灸治疗,2组连续治疗4周。比较2组治疗前后简化McGill疼痛问卷评分、Constant-Murley肩关节功能量表评分、肩关节活动度,评估2组临床疗效及不良反应发生情况。结果:治疗4周后,治疗组总有效率91.84%,高于对照组75.51%,差异有统计学意义(P<0.05)。治疗4周后,2组简化McGill疼痛问卷评分低于治疗前(P<0.05),Constant-Murley肩关节功能量表维度(日常生活活动、疼痛、关节活动度、肌力)评分及肩关节前屈、后伸、外展活动度均较治疗前升高(P<0.05),且治疗组简化McGill疼痛问卷评分低于对照组(P<0.05),Constant-Murley肩关节功能量表各维度评分及肩关节前屈、后伸、外展活动度高于对照组(P<0.05)。2组治疗期间均未出现明显不良反应。结论:温针灸联合温通膏外敷治疗寒湿痹阻型肩周炎疗效显著,能够缓解患者疼痛,恢复肩关节功能,增大肩关节活动度,安全性较高。Objective:To observe the clinical effect of warming-needle moxibustion combined with external application with Wentong Ointment on scapulohumeral periarthritis of cold-damp obstruction type.Methods:A total of 98 cases of patients with scapulohumeral periarthritis of cold-damp obstruction type were divided into the treatment group and the control group according to the random number table method,with 49 cases in each group.The control group was treated with external application with Wentong Ointment,and the treatment group was additionally treated with warming-needle moxibustion based on the treatment of the control group.Both groups were treated for 4 weeks.Simplified McGill Pain Questionnaire scores,Constant-Murley Shoulder Joint Function Scale scores and shoulder range of motion were compared between the two groups before and after treatment;the clinical effects and the incidence of adverse reactions in the two groups were evaluated.Results:After 4 weeks of treatment,the total effective rate was 91.84% in the treatment group,higher than that of 75.51% in the control group,the difference being significant(P<0.05).After 4 weeks of treatment,the scores of simplified McGill Pain Questionnaire in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of Constant-Murley Shoulder Joint Function Scale(activities of daily living,pain,range of motion,muscle strength) and the degree of shoulder flexion,extension and abduction were increased when compared with those before treatment(P<0.05);the score of simplified McGill Pain Questionnaire in the treatment group was lower than that in the control group(P<0.05),and the scores of all dimensions of the Constant-Murley Shoulder Joint Function Scale and the degree of shoulder flexion,extension and abduction were higher than those in the control group(P<0.05).During treatment,there was no obvious adverse reaction in the two groups.Conclusion:Warming-needle moxibustion combined with external application with Wentong Ointment has a significa
