超声脂肪分数评估代谢相关脂肪性肝病的初步研究

摘　　要：目的探讨以肝脏组织病理学检查作为金标准,超声脂肪分数(UDFF)无创性评估代谢相关脂肪性肝病(MAFLD)不同程度肝脏脂肪变性的诊断效能。方法前瞻性地收集行肝脏组织病理学检查,确诊为MAFLD的患者32例。通过测量每个MAFLD患者的UDFF,分析其所对应的不同病理学等级,进而评估UDFF与不同病理等级的相关性。最后,采用受试者工作特征(ROC)曲线评估UDFF对MAFLD肝脏脂肪变性程度的诊断效能。结果UDFF与MAFLD患者的肝脏脂肪变性程度呈正相关(r=0.59,P<0.001)。MAFLD患者的UDFF在不同程度肝脏脂肪变性之间的差异具有统计学意义(P<0.05)。UDFF诊断中度及以上肝脏脂肪变性截断值为9.6%,曲线下面积为0.794,灵敏度为77.8%,特异度为81.0%。结论UDFF对MAFLD不同程度肝脏脂肪变性的评估表现出较好的诊断效能。Objective To explore the diagnostic efficacy of ultrasound-derived fat fraction(UDFF)in non-invasive evaluating the degrees of hepatic steatosis in metabolic associated fatty liver disease(MAFLD)by using liver histopathological examination as gold standard.Methods A total of 32 patients diagnosed with MAFLD by liver histopathological examination were prospectively collected.The UDFF of each MAFLD patient was measured and the pathological grade was analyzed,and the correlation between UDFF and pathological grade was evaluated.Then the diagnostic efficacy of UDFF in MAFLD was evaluated by receiver operating characteristic(ROC)curve.Results The UDFF was positive correlated to the degree of hepatic steatosis in patients with MAFLD(r=0.59,P<0.001).There were significant difference in UDFF values of patients with MAFLD among different degrees of hepatic steatosis.The cut-off value of UDFF for moderate and above degree of hepatic steatosis was 9.6%,the area under the cure was 0.794,the sensitivity was 77.8%,and the specificity was 81.0%.Conclusions UDFF has shown good diagnostic efficacy in hepatic steatosis degree evaluation in MAFLD.
