超声在类风湿性关节炎腕关节病变中的应用价值

摘　　要：目的探讨超声在类风湿性关节炎腕关节病变中的应用价值。方法对86例类风湿性关节炎患者分别采用肌骨超声和磁共振成像检查评估患者腕关节骨质侵蚀、骨质疏松、关节腔积液、腱鞘炎等病变情况,检测患者腕关节滑膜血流分级。以磁共振成像检查结果为标准,分析肌骨超声对检查类风湿性关节炎腕关节病变的应用价值。结果病例组腕关节滑膜平均厚度较对照组显著增厚(P<0.05),对照组和病例组腕关节滑膜血流分级差异具有统计学意义(P<0.05)。以磁共振成像检查结果为标准,肌骨超声检查对腕关节病变的灵敏度、特异度、阳性预测值、准确度和Kappa值分别为87.18%、75.00%、91.43%、86.05%和0.435。磁共振成像和肌骨超声对病例组滑膜增厚、骨质侵蚀、骨质疏松、关节腔积液以及腱鞘炎的检出率基本一致(P>0.05)。超声与磁共振成像对腕关节滑膜血流分级的检出率基本一致(P>0.05)。结论肌骨超声在类风湿性关节炎腕关节病变检查中具有较高的临床价值,可用于类风湿性关节炎腕关节病变的筛选。Objective To investigate the clinical value of ultrasound in the wrist joint lesion of rheumatoid arthritis.Methods A total of 86patients with rheumatoid arthritis were examined by musculoskeletal ultrasound and magnetic resonance imaging(MRI)to evaluate the changes of bone erosion,osteoporosis,joint cavity effusion and tenosynovitis.00000We detected the grade of synovial blood flow in the wrist.The clinical value of musculoskeletal ultrasound in the examination of early wrist joint lesions of in rheumatoid arthritis were analyzed on the golden standards of MRI.Results Based on the results of MRI,the sensitivity,specificity,positive prediction rate,accuracy and Kappa value of musculoskeletal ultrasonography for early wrist lesions were respectively 87.18%,75.00%,91.43%,86.05%and 0.435.The detection rates of synovial thickening,bone erosion,osteoporosis,joint effusion and tenosynovitis by MRI and musculoskeletal ultrasound were basically consistent(P>0.05).The results of ultrasound examination of wrist synovial blood flow grading were basically consistent with that of MRI(P>0.05).Conclusions Musculoskeletal ultrasound has a high clinical value in the examination of the lesions of the wrist of rheumatoid arthritis.It can be used to screen the early lesions of the wrist of rheumatoid arthritis.
