超声及绘图定位在评估自体动静脉内瘘成熟及术后狭窄中的应用价值

摘　　要：目的探讨超声及绘图定位在自体动静脉内瘘(AVF)术后成熟度及血管狭窄评估中的应用价值。方法将107例AVF术后患者根据临床使用情况分为成熟组及未成熟组。对比两组术后血流动力学参数,采用受试者工作特征(ROC)曲线评估超声血流动力学参数对内瘘成熟的诊断价值。选取AVF术后出现血管狭窄并进行经皮血管腔内成形术(PTA)的患者32例,对比术前进行超声辅助下绘图定位组与未进行绘图定位组的手术成功率及手术时间。结果成熟组肱动脉流量、桡动脉内径、桡动脉流量大于未成熟组(P<0.05);两组肱动脉内径差异无统计学意义(P>0.05)。ROC曲线结果显示肱动脉流量和桡动脉流量对内瘘成熟的诊断价值较高,灵敏度及特异度分别为90.5%/81.8%、90.5%/78.8%。超声辅以绘图定位组PTA手术时间较未绘图定位组缩短(P<0.05)。结论超声辅以绘图技术在AVF术后成熟度及狭窄的评估中具有重要应用价值。Objective To explore the application value of ultrasound-assisted drawing localization in the evaluation of maturity and vascular stenosis of autogenous arteriovenous fistula(AVF).Methods A total of 107 patients after AVF were divided into the mature group and the immature group according to the clinical evaluation.The postoperative hemodynamic parameters of the two groups were compared,and the receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of ultrasonic hemodynamic parameters for fistula maturation.32 patients with vascular stenosis after AVF who underwent percutaneous transluminal angioplasty(PTA)were selected,to compare the success rate and operation time between the groups with ultrasound-assisted drawing localization and the group without drawing localization.Results The brachial artery flow,radial artery diameter and radial artery flow in the mature group were higher than those in the immature group(P<0.05),and there was no significant difference in the brachial artery diameter between the two groups(P>0.05).The results of ROC showed that brachial artery flow and radial artery flow had a high diagnostic value for fistula maturation,with the sensitivity and specificity of 90.5%/81.8%and 90.5%/78.8%.The operation time of the group with drawing localization was shorter than that without(P<0.05).Conclusions Ultrasonography assisted drawing technique has important application value in the evaluation of postoperative maturity and stenosis of AVF.
