乳腺韧带样型纤维瘤病的超声表现与临床特征

摘　　要：目的总结乳腺韧带样型纤维瘤病的超声图像与临床特征,提高临床及超声医师对该疾病的认识。方法回顾分析15例乳腺韧带样型纤维瘤病患者的临床资料及超声图像,分析声像图特征。结果15例患者共27个病灶均在手术中切除,病理证实均为乳腺韧带样型纤维瘤病。73.3%患者年龄≤40岁,88.9%的病灶>2 cm,66.7%的病灶位于腺体层内并累及后板层筋膜、深筋膜或胸大肌,92.6%病灶超声表现为内部回声不均,呈交织状,96.3%病灶平行生长,77.8%病灶侧方成角,全部病灶均无钙化及导管结构扭曲表现,腋下均无明显增大淋巴结。70.4%病灶血流Adler分级0~Ⅰ级。复发型病灶与原发病灶超声表现相似。结论超声诊断乳腺韧带样型纤维瘤病有一定的特征性表现,结合患者临床资料能够提高临床及超声医师对该病的诊断率。Objective To summarize the ultrasound images and clinical features of breast desmoid-type fibromatosis,and to improve the understanding of the disease by clinical physicians and sonographers.Methods The clinical data and ultrasound images of 15 patients with desmoid-type fibromatosis of the breast were analyzed retrospectively.Results A total of 27 lesions in 15 patients were resected during surgery,and the pathology of all lesions was desmoid-type fibromatosis of the breast.73.3%of the patients were younger than 40 years old,88.9%of the lesions were larger than 2cm,66.7%of the lesions were in the gland layer and involving posterior fascia、deep fascia or pectoralis major muscle,92.6%of the lesions showed uneven internal echo and interlaced,96.3%of the lesions showed parallel growth,77.8%of the lesions were angled laterally,all the lesions showed no calcification and duct structure distortion,and no obvious enlarged lymph nodes were found under the armpit.70.4%of the lesions blood flow Adler were grade 0-Ⅰ.The ultrasonic findings of recurrent lesions were similar to those of primary lesions.Conclusions Ultrasound has certain characteristics in the diagnosis of breast desmoid-type fibromatosis and combining with the clinical data of patients can improve the diagnostic rate of the disease for clinical physicians and sonographers.
