热毒理论在慢性病热毒证康复治疗中的应用探讨

摘　　要：阐述“热毒”现代研究病理基础、慢性病热毒特点以及“热毒”在慢性病发生发展过程中的形成机制及重要影响。根据丁书文教授治疗热毒证提出的清解、排泄和调补治疗方案,综合中医治病求本、扶正祛邪的治疗原则,从“清与解”立法,辨证选用黄连解毒汤、四妙勇安汤等方药,达到清热解毒、透邪外出的目的。“排与泄”选用大黄黄连泻心汤等方药利尿燥湿、排毒泄热,选甘寒清润、甘酸咸寒之品,辅佐清热解毒方药益气滋阴,对机体进行“调与补”。除中药膏方外,还可辨证选用针刺特定穴位、食疗茶饮和运动发汗等中医康复治疗手段,促进慢性热毒病证的康复。“热毒”扩展了慢性病后期的中医病机,可为慢性疾病的康复治疗及研究提供借鉴和支持。This paper elaborated the modern pathological basis of heat toxin,the characteristics of heat toxin in chronic diseases,and the formation mechanism and important influence of heat toxin in the occurrence and development of chronic diseases.According to the therapeutic scheme of clearing,draining,regulating and replenishing proposed by Professor DING Shuwen for the treatment of heat toxin syndrome,combined with the therapeutic principle of traditional Chinese medicine(TCM)to treat diseases by seeking the root cause,reinforce healthy qi and remove pathogen,treatment methods of“clearing and resolving”were used and Huanglian Jiedu Decoction(黄连解毒汤),Simiao Yong’an Decoction(四妙勇安汤)and other formulas were selected based on syndrome differentiation to achieve the purpose of clearing heat,removing toxin and expelling pathogen.For“draining and discharging”,Dahuang Huanglian Xiexin Decoction(大黄黄连泻心汤)and other formulas were selected for promoting urination,drying dampness,removing toxin and discharging heat.Cooling and moist medicinals sweet in flavor and cold in property,or sweet,sour,salty in flavor and cold in property were selected to assist the formulas for heat-clearing and toxin-removing to replenish qi and nourish yin,and“regulate and replenish”the body.In addition to Chinese medicinal paste,acupuncture at specific points,diet tea and exercise sweating and other therapeutic methods of rehabilitation treatment in TCM were also selected based on syndrome differentiation to promote the rehabilitation of chronic heat toxin syndrome.“Heat toxin”expanded the TCM pathogenesis in the late stage of chronic diseases,which might provide reference and support for the rehabilitation treatment and research of chronic diseases.
