膝关节解剖因素与前交叉韧带磁共振成像信号强度相关性分析

摘  要:目的:探讨健康人群膝关节解剖因素与前交叉韧带(anterior cruciate ligament,ACL)在磁共振成像(magnetic resonance imaging,MRI)上信号强度的关系。方法:2023年1~5月招募健康受试者100例,男性54例、女性46例,年龄36.87±7.21(20~50)岁。通过MRI测量胫骨平台骨性后倾角、胫骨平台软组织后倾角、半月板-胫骨平台角(meniscus-bone angle,MBA)、髁间窝宽度指数(notch width index,NWI)、髁间嵴高度及ACL信号强度。首先分析性别、年龄、胫骨平台骨性后倾角、胫骨平台软组织后倾角、MBA、NWI、髁间嵴高度与ACL信号强度是否具有相关性,然后采用多元线性回归进一步分析相关因素对健康人群ACL信号强度的影响。结果:性别、外侧MBA与ACL的信号与噪音比值(the signal to noise quotient,SNQ)均呈现显著相关性(r=-0.283,P=0.004;r=-0.213,P=0.033),而年龄、内侧胫骨平台骨性后倾角、内侧胫骨平台软组织后倾角、外侧胫骨平台骨性后倾角、外侧胫骨平台软组织后倾角、内侧MBA、NWI、内侧髁间嵴高度、外侧髁间嵴高度与ACL的SNQ相关性均无统计学意义(P>0.05)。进行多重线性回归分析,结果显示女性(t=-2.907,P=0.005)和外侧MBA(t=-2.126,P=0.036)均为健康人群ACL在MRI上信号强度的影响因素。结论:在健康人群中,外侧MBA、性别与ACL的SNQ均存在显著相关性,且外侧MBA越大,ACL信号强度越低,女性ACL信号强度较男性更低。Objective To explore the correlation of anatomic factors of the knee to the anterior cruci⁃ate ligament(ACL)signal intensity on magnetic resonance imaging(MRI)in the healthy population.Methods In the first 5 months of 2023,100 healthy subjects ranging in age from 20 to 50 were re⁃cruited,including 54 males and 46 females and aged 36.87±7.21 years.They were measured the posterior tibial slope,meniscus slope angle,meniscus-bone angle(MBA),notch width index(NWI),the height of tibial spine and ACL signal intensity by using MRI.Then,the relationship between the ACL signal intensity and other measurements was analyzed,and the factors affecting the ACL signal in⁃tensity were further explored using the multiple linear regression.Results Gender and lateral MBA were significantly correlated to the signal to noise quotient(SNQ)of ACL(r=-0.283,P=0.004;r=-0.213,P=0.033),while other factors such as age,the medial posterior tibial slope,medial meniscus slope angle,lateral posterior tibial slope,lateral meniscus slope angle,medial MBA,and NWI,as well as the heights of medial and lateral tibial intercondylar eminence were not significantly correlated to the latter(P>0.05).Moreover,multiple linear regression analysis found that both female(t=-2.907,P=0.005)and lateral MBA(t=-2.126,P=0.036)were influencing factors for the signal intensi⁃ty of ACL on MRI.Conclusion In healthy population,lateral MBA and gender are significantly corre⁃lated to SNQ of ACL.To be more specific,the greater the lateral MBA,the lower the ACL signal in⁃tensity,especially among the female.
