益气通络、健脾补肾法联合穴位贴敷治疗慢阻肺稳定期的临床疗效分析

摘　　要：目的:探究益气通络、健脾补肾法联合穴位贴敷治疗慢阻肺(COPD)稳定期的临床疗效。方法:选取我院2022年1月~2023年9月收治的COPD稳定期患者180例。按照照随机数字表法将患者分为穴位贴敷组、中药治疗组和联合治疗组,各60例,穴位贴敷组为西医治疗+穴位贴敷,中药治疗组为西医治疗+益气通络、健脾补肾法,联合治疗组为西医治疗+穴位贴敷+益气通络、健脾补肾法,比较治疗前后中医症候积分、肺功能[肺活量占预计值百分比(FVC%pred)、FEVl/FVC、一氧化碳弥散量占预计值百分比(DLCO%pred)]、炎症指标[白介素(IL)-8、肿瘤坏死因子(TNF)-α、C-反应蛋白(CRP)]、心血管舒缩功能因子[内皮素(ET)、内皮素源(NO)]及生活质量(BODE评分系统)差异。结果:治疗后,三组各中医证候积分、各血清炎症指标与ET水平、mMRC等级及BODE指数均显著降低,且联合治疗组显著低于穴位敷贴组及中药治疗组(P<0.05);治疗后,三组各肺功能指标、血清NO水平、第1秒用力呼气容积占预计值百分比(FEVl%pred)及6WMT均显著升高,且联合治疗组显著高于另两组(P<0.05)。结论:益气通络、健脾补肾法联合穴位贴敷法治疗COPD稳定期患者,能有效改善临床症状,减轻炎性反应,增强肺功能,提高患者预后生活质量。Objective:To explore the clinical efficacy of Yiqi Tongluo and Jianpi Bushen decoction combined with acupoint application on stage chronic obstructive pulmonary disease(COPD).Methods:180patients with stable COPD were selected from our hospital from January 2022to September 2023.According to the random number table method,the patients were divided into acupoint application group,traditional Chinese medicine treatment group and combined treatment group,with 60cases in each group.The acupoint application group was given western medicine treatment+acupoint application,the traditional Chinese medicine treatment group was given western medicine treatment+Yiqi Tongluo and Jianpi Bushen decoction,and the combined treatment group was treated with western medicine treatment+acupoint application+Yiqi Tongluo and Jianpi Bushen decoction.TCM syndromes scores, pulmonary function [forced vital capacity as a percentage of predicted value (FVC% pred), FEVl / FVC, carbonmonoxide dispersion as a percentage of predicted value (DLCO% pred) ], inflammatory indicators [interleukin (IL) -8,tumor necrosis factor (TNF) -α, C-reactive protein (CRP) ], cardiovascular systolic-diastolic function factors [endothelin(ET), nitric oxide (NO) ] and quality of life (BODE score system) were compared before and after treatment. Results: Aftertreatment, the TCM syndromes scores, serum inflammatory indicators, ET, mMRC grading and BODE index were significantlydecreased in the three groups, and the above indicators in combined treatment group were significantly lower than those in acupointapplication group and traditional Chinese medicine treatment group (P<0. 05). After treatment, the pulmonary function indicators,serum NO level, forced expiratory volume in the first second as a percentage of predicted value (FEVl% pred) and6WMT were significantly increased in the three groups, and the above indicators were significantly higher in combined treatmentgroup than the other two groups (P<0. 05). Conclusion: Yiqi Tongluo and Jianpi Bushen decoction combine
