益气止鼽汤辅助糠酸莫米松鼻喷雾剂治疗变应性鼻炎疗效及对CCL17、YKL-40的变化研究

摘　　要：目的研究益气止鼽汤辅助糠酸莫米松鼻喷雾剂喷鼻治疗变应性鼻炎疗效及对患者CC趋化因子配体17(CC chemokine ligand 17,CCL17)、甲壳质酶蛋白-40(chitinase protein-40,YKL-40)的变化影响。方法选取济南市人民医院2020年12月—2021年12月收治的变应性鼻炎患者88例,采用随机数字法分为两组(每组44例)。鼻喷雾剂组给予糠酸莫米松鼻喷雾剂喷鼻治疗,益气止鼽汤组在鼻喷雾剂基础上给予益气止鼽汤治疗。比较两组患者治疗4周后的疗效,检测两组CCL17、YKL-40等相关因子的差异,统计两组3个月后复发率。结果益气止鼽汤组总有效率为90.91%(40/44)高于鼻喷雾剂组的75.00%(33/44),差异有统计学意义(P<0.05)。两组治疗前CCL17、YKL-40、嗜酸粒细胞阳离子蛋白(Eosinophil cationic protein,ECP)、炎症因子组间比较,差异无统计学意义(P>0.05)。与治疗前比较,两组CCL17、YKL-40、ECP、白细胞介素-4(interleukin-4,IL-4)、白细胞介素-17(interleukin-17,IL-17)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)治疗4周后降低,且益气止鼽汤组低于鼻喷雾剂组(P<0.05)。益气止鼽汤组3个月后复发率为6.82%(3/44)低于鼻喷雾剂组的22.73%(10/44),差异有统计学意义(P<0.05)。结论益气止鼽汤辅助糠酸莫米松鼻喷雾剂喷鼻治疗变应性鼻炎可减轻炎症反应,降低CCL17、YKL-40、ECP的表达水平,提高疗效,降低复发率。Objective To study the influence of Yiqi Zhiqiu Decoction(益气止鼽汤)assisted momesone furoate nasal spray in allergic rhinitis on CC chemokine ligand 17(CCL17)and chitinase protein-40(YKL-40).Methods A total of 88 patients with allergic rhinitis admitted to Jinan Peoples Hospital from December 2020 to December 2021 were selected and divided into two groups(44 cases in each group)by random number method.The nasal spray group was treated with mometasone furoate nasal spray.The Yiqi Zhiqiu Decoction group combined with Yiqi Zhiqiu Decoction on the basis of mometasone furoate nasal spray.The efficacy of the two groups after 4 weeks of treatment was compared,and the differences of CCL17,YKL-40 and other related factors between the two groups were detected,and the recurrence rate of the two groups after 3 months was analyzed.Results The total effective rate of Yiqi Zhiqiu Decoction group was 90.91%(40/44),higher than 75.00%(33/44)in the nasal spray group(P<0.05).Before treatment,there was no statistical significance in CCL17,YKL-40,eosinophilic cationic protein(ECP)or inflammatory factors between the two groups(P>0.05).Compared with those before treatment,the levels of CCL17,YKL-40,ECP,interleukin-4(IL-4),interleukin-17(IL-17)and tumor necrosis factor-α(TNF-α)were decreased in both groups after 4 weeks of treatment,and the levels were significantly lower in the Yiqi Zhiqiu Decoction group than those in the nasal spray group(P<0.05).The recurrent rate of the Yiqi Zhiqiu Decoction group in 3 months was 6.82%(3/44),which was significantly lower than that in nasal spray group[22.73%(10/44)](P<0.05).Conclusion Yiqi Zhiqiu Decoction assisted mometasone fu⁃roate nasal spray in the treatment of allergic rhinitis can alleviate inflammation,reduce the levels of CCL17,YKL-40 and ECP,improve the curative effect and reduce the recurrence rate.
