摘 要:目的 比较多排CT头颈部血管造影中低剂量与正常剂量对比剂的应用效果。方法 选取2020年1月至2023年1月收治的82例头颈部CT血管造影(CTA)患者,回顾性分析临床资料,根据CTA扫描对比剂剂量将82例患者分为低剂量组与正常剂量组,分别为38例、44例。入组者均行头颈部CTA检查,低剂量组将剂量为60 ml碘海醇注射液加20 ml浓度为0.9%氯化钠注射液进行注射,正常剂量组将剂量70 ml碘海醇注射液加30 ml浓度为0.9%氯化钠注射液进行注射,对比2组图像质量、头颈部CT值、头颈部血管成像显影CT值及残留显影CT值。结果 低剂量组图像质量优良率(87%)较正常剂量组(68%)更高(P<0.05);低剂量组左右颈总动脉(CCA)起始部、左右颈外动脉(ECA)起始部、右锁骨下动脉(SA)起始部、左颈动脉(ICA)虹吸部、左右V2起始部、左右CCA环状软骨及左右ICA起始部CT值较正常剂量组更低(P<0.05);低剂量组颈总动脉、大脑中动脉M1段、颈动脉分叉及主动脉弓CT值较正常剂量组更低(P<0.05);低剂量组颈内静脉、头臂静脉、上腔静脉及锁骨下静脉CT值较正常剂量组更低(P<0.05)。结论 头颈部CTA检查应用低剂量对比剂,可促进图像质量提高,并减少辐射剂量。Objective To compare the application effect of low-dose and normal-dose contrast in multi-row CT head and neck angiography.Methods Eighty-two head and neck computed tomography angiography(CTA)patients admitted to our hospital from January 2020 to January 2023 were selected,and the clinical data were retrospectively analyzed,and 82 patients were divided into the low-dose group and normal-dose group by the dose of the contrast agent for CTA scanning,which was 38 cases and 44 cases,respectively.All the enrolled patients underwent head and neck CTA examination,and the low-dose group was injected with a dose of 60 ml iohexol injection plus 20 ml of 0.9%sodium chloride injection,and the normal-dose group was injected with a dose of 70 ml iohexol injection plus 30 ml of 0.9%sodium chloride injection,and the two groups were compared concerning the image quality,the head and neck CT value,the head and neck angiographic contrast CT value and the residual contrast CT values.Results The image quality excellence rate(87%)was higher in the low-dose group than the in the normal-dose group(68%)(P<0.05).The CT values of the left and right CCA initiation,the left and right ECA initiation,the right SA initiation,the left ICA siphon,the left and right V2 initiation,the left and right CCA annular cartilages,and the left and right ICA initiation were lower in the low-dose group than in the normal-dose group(P<0.05).The CT values of the common carotid artery,M1 segment of the middle cerebral artery,carotid bifurcation,and aortic arch were lower in the low-dose group than in the normal-dose group(P<0.05).The CT values of internal jugular vein,cephalic brachial vein,superior vena cava and subclavian vein in the low-dose group were lower in the low-dose group than in the normal-dose group(P<0.05).Conclusion The application of low-dose contrast agents in head and neck CTA examination can promote the improvement of image quality and reduce the radiation dose.
