浅析桂枝加附子汤治疗多汗症

摘　　要：多汗症是指全身或局部皮肤出汗异常多的疾病,为当今社会常见多发疾病。文章仅从桂枝加附子汤入手浅谈中医对多汗症的治疗,内容包括多汗症诊治概况、桂枝加附子汤治疗多汗症经文记载、方义解析、临床病例应用等。中医认为多汗症是由于阴阳失调,营卫失和,腠理不固而致汗液外泄失常的病症,包括自汗、盗汗。多汗症病机总属脏腑阴阳失调,营卫失和,不离虚、实二纲。中医治疗多汗症多审证求因,辨证论治。西医学者则认为多汗症是由于中枢神经系统调节功能紊乱使交感神经系统发出过多、过强的兴奋刺激,通过乙酰胆碱令汗腺大量分泌汗液而造成多汗,西医治疗方式多种,但效果欠理想且不良反应不容忽视。中医不乏关于桂枝加附子汤治疗多汗症的经文记载。桂枝加附子汤组方为桂枝汤加附子一枚,其中炙甘草的用量在桂枝汤基础上加至三两,使炙甘草、桂枝、芍药等比配伍,具有调和营卫,扶阳固表,复阳敛液之功效。临症之时,不应局限于汗证、痹病、痛证等,应遵循“治病求本、异病同治”中医之精髓,病虽有异,究其辨证,凡符合阳气虚弱、阴亦不足,表证未除之病机者,皆可大胆运用此方。Hyperhidrosis refers to the abnormal excessive sweating of the whole body or local skin,which is a common and frequent disease in today’s society.This paper discusses Guizhi and Fuzi decoction(桂枝加附子汤)in the treatment of hyperhidrosis.The contents include the diagnosis and treatment of hyperhidrosis,scription records of Guizhi and Fuzi decoction in the treatment of hyperhidrosis,the interpretation of the prescription,and the application of clinical cases.TCM believes that hyperhidrosis is due to Yin(阴)and Yang(阳)imbalance,Qing(营)and Wei(卫)disharmony,and insecurity of the interstices,which leads to sweat leakage disorders,including spontaneous sweating and night sweats.Hyperhidrosis pathogenesis is Yin and Yang imbalance of Zangfu vislera(脏腑),Ying and Wei disharmony,which can be included in deficiency and excess.TCM focuses on examining the causes,syndrome differentiation and treatment in the treatment of hyperhidrosis.Western medicine scholars believe that hyperhidrosis is due to the disorder of central nervous system regulation,which makes the sympathetic nervous system issue too much and too strong stimulation,and causes hyperhidrosis due to excessive secretion of sweat by sweat glands through the medium of acetylcholine.Western medicine has a variety of treatment methods,but the effect is not ideal and the side effects cannot be ignored.There is no shortage of TCM prescription records on Guizhi and Fuzi decoction in the treatment of hyperhidrosis.Guizhi and Fuzi decoction adds Fuzi(Radix Aconiti Lateralis Preparata)on the basis of Guizhi decoction(桂枝汤),in which the amount of Zhigancao(prepared licorice)is added to 3 Liang,so that Zhigancao,Guizhi(cassia twig)and Shaoyao(Radix Paeoniae Alba)are matched in equal proportion.with the effects of regulating Ying and Wei,supporting Yang and consolidating the surface,and restoring Yang and collecting liquid.In treating the disease,it should not be limited to sweating syndrome,arthralgia,pain,etc.,but should follow the essence of TCM of�
