摘  要:目的:观察枸橼酸咖啡因联合经鼻持续气道正压通气治疗呼吸窘迫综合征早产儿的效果。方法:选取2021年5月至2022年8月该院收治的106例呼吸窘迫综合征早产儿进行前瞻性研究,按照随机数字表法分为研究组与对照组各53例。对照组采用经鼻持续气道正压通气治疗,研究组在对照组基础上联用枸橼酸咖啡因注射液治疗,比较两组治疗总有效率、血气指标[动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))]水平和不良反应发生率。结果:研究组治疗总有效率为96.23%,明显高于对照组的84.91%,差异有统计学意义(P<0.05);治疗7 d后,两组PaO_(2)水平高于治疗前,且研究组高于对照组,两组PaCO_(2)水平低于治疗前,且研究组低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:枸橼酸咖啡因联合经鼻持续气道正压通气治疗呼吸窘迫综合征早产儿可提高治疗总有效率,改善血气指标水平,效果优于单纯经鼻持续气道正压通气治疗。Objective:To observe effects of Caffeine citrate combined with nasal continuous positive airway pressure ventilation in treatment of premature infants with respiratory distress syndrome.Methods:A prospective study was conducted on 106 premature infants with respiratory distress syndrome admitted to the hospital from May 2021 to August 2022.According to the random number table method,they were divided into study group and control group,53 cases in each group.The control group was treated with nasal continuous positive airway pressure ventilation,while the study group was treated with Caffeine citrate injection on the basis of that of the control group.The total effective rate,the blood gas index[arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))]levels,the Bayley scales of infant and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the study group was 96.23%,which was significantly higher than 84.91%of the control group,and the difference was statistically significant(P<0.05).After 7 days of treatment,the PaO_(2) levels of the two groups were higher than those before treatment,and that in the study group was higher than that in the control group;the PaCO_(2) level of the two groups was lower than that before treatment,and that in the study group was lower than that in the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Caffeine citrate combined with nasal continuous positive airway pressure ventilation in the treatment of the premature infants with respiratory distress syndrome can improve the total effective rate and improve the levels of blood gas analysis indexes.Moreover,it is superior to single nasal continuous positive airway pressure ventilation treatment.
