卡瑞利珠单抗致昏迷罕见不良反应1例

摘　　要：回顾性分析1例卡瑞利珠单抗致昏迷罕见不良反应的病例。患者,男, 73岁,诊断为右肺恶性肿瘤,既往行多程多线治疗。患者于2021年12月12日08:43行卡瑞利珠单抗200 mg, d_(1), q.2周.第3程免疫治疗,卡瑞利珠单抗输注10 min后,突发意识丧失、呼之不应,考虑系药物引起的不良反应,立即停止输注卡瑞利珠单抗,并立即更换生理盐水250 ml静脉滴注,测指间血糖6.1 mmol/L,心电监护示:血压118/74 mm Hg(1 mm Hg=0.133 kPa),呼吸20次/min,心率87次/min,患者昏迷状态持续约3 min, 08:56患者症状明显缓解,意识恢复,对答切题。This paper retrospectively analyze a case with rare adverse reaction of coma induced by camrelizumab.Patient,male,73 years old,diagnosed with malignant tumor of the right lung,previously treated with multiple courses and lines of therapy.On December 12,2021,at 08:43,the patient underwent a third course of immunotherapy with 200 mg d1 q.2 weeks of camrelizumab.After a 10 min infusion of camrelizumab,the patient suddenly lost consciousness and was unable to respond.Considering the adverse reactions caused by the drug,the infusion of camrelizumab was stopped immediately,and 250 ml of normal saline was immediately replaced by intravenous infusion.Interfinger blood glucose was measured at 6.1 mmol/L,and ECG monitoring showed that blood pressure 118/74 mm Hg(1 mm Hg=0.133 kPa),breathing rate 20 times/min,heart rate 87 beats/min,the patient's coma lasted for about 3 min.At 08:56,the patient's symptoms significantly improved,consciousness recovered,and answered the questions accurately.
