自拟通管汤联合坐浴治疗输卵管性不孕症的临床观察

摘　　要：目的探讨输卵管性不孕症患者应用自拟通管汤联合坐浴治疗的临床效果。方法选取2020年10月至2022年10月宜春市中医院收治的60例输卵管性不孕症患者作为研究对象,采用随机数字表法分为对照组(30例)和治疗组(30例)。对照组患者采用常规输卵管疏通治疗,治疗组患者在对照组治疗方法的基础上加用自拟通管汤联合坐浴治疗。比较两组患者的临床疗效、中医证候积分、局部体征积分、炎症因子水平、子宫动脉血流参数、输卵管通畅程度、自然受孕率。结果治疗组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗前的中医证候积分、局部体征积分、肿瘤坏死因子-α(TNF-α)白细胞介素-6(IL-6)、白细胞介素-8(IL-8)子宫动脉阻力指数(RI)、收缩期峰值流速/舒张末期流速(S/D)比较,差异无统计学意义(P>0.05);两组患者治疗后的中医证候积分、局部体征积分、TNF-α、IL-6、IL-8、子宫动脉RI、S/D均低于本组治疗前,且治疗组的以上各项均低于对照组,差异有统计学意义(P<0.05)。治疗组治疗后的输卵管通畅程度优于对照组,自然受孕率高于对照组,差异有统计学意义(P<0.05)。结论自拟通管汤联合坐浴治疗输卵管性不孕症可改善子宫动脉血流及临床症状、体征,降低炎症因子水平,利于输卵管的复通,提高自然受孕率,值得临床推广。Objective To explore the clinical effect of self-made Tongguan Decoction combined with sitz bath in the treat-ment of tubal infertility.Methods A total of 60 patients with tubal infertility admitted to Yichun Hospital of Traditional Chi-nese Medicnine from October 2020 to October 2022 were selected as the study subjects.They were divided into control group(30 cases)and treatment group(30 cases)according to random number table method.The patients in the control group were treated with routine tubal patency therapy,while the patients in the treatment group were treated with self-made Tong-guan Decoction combined with sitz bath therapy on the basis of the treatment of the control group.The clinical fficacy,tra-ditional Chinese medicine syndrome scores,local signs scores,inflammatory factor levels,uterine artery blood flow parame-ters,tubal patency,and natural conception rate between the two groups were compared.Results The total effective rate of the treatment group was higher than that of the control group,with a statistically significant difference(P<0.05).Before treatment,there were no statistically significant differences in traditional Chinese medicine syndrome score,local sign score,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),uterine artery resistance index(RI),peak systolic velocity/end diastolic velocity(S/D)between the two groups(P>0.05).After treatment,the traditional Chinese medicine syndrome score,local sign score,TNF-α,IL-6,IL-8,uterine artery RI,S/D of the two groups were lower than those before treatment,and the above in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the tubal patency of the treatment group was better than that of the control group,and the natural pregnancy rate of the treatment group was higher than that of the control group,and the differ-ences were statistically significant(P<0.05).Conclusion The combination of self-made Tongguan Decoction and sitz bath can impro
