显微精索静脉曲张手术联合中西医治疗非梗阻性无精子症配偶自然妊娠活产一例

摘　　要：本文报道1例患有非梗阻性无精子症、精索静脉曲张的男性不育患者,通过显微精索静脉结扎手术(术前行精索动脉监测评估保护精索动脉),术中结扎左侧蔓状精索静脉6根,结扎精索蔓状静脉,保护精索动脉3根,术后联合中西医药物治疗方案(来曲唑+仙鹿口服液),最终获得足够数量的射出精子,使得其配偶自然妊娠并活产1名女婴。本案例表明,对于此类患者,除了辅助生殖技术,微创泌尿外科手术联合中西医药物治疗也是一种有效的治疗策略。这种方法不仅能减轻患者的手术创伤,还能降低经济成本,有助于促进优生优育。This paper reports a male infertility patient with non-obstructive azoospermia and varicocele who underwent microsurgical varicocelectomy(preoperative monitoring and evaluation of the spermatic artery to protect the spermatic artery).During the operation,6 left pampiniform spermatic veins were ligated,3 pampiniform veins were ligated and the spermatic arteries were protected.After the surgery,the patient was treated with traditional Chinese and western medicine(Letrozole+Xianlu Oral Liquid),and enough ejaculated sperm was obtained,so that his spouse had a natural pregnancy and live birth of a female infant.This case shows that in addition to assisted reproductive technology,minimally invasive urological surgery combined with traditional Chinese and western medicine treatment is also an effective treatment strategy for such patients.This method can not only reduce the surgical trauma of the patient,but also reduce the economic cost and help to promote eugenics.
