摘  要:目的 利用能谱CT物质定量分析功能,通过与双能X线吸收仪比较,探讨HAP-水、钙-水、及HAP-Fat基物质成像测量骨密度的可能性。方法 选取在我院接受腹部CT检查的成年女性146例作为观察组,年龄21~89岁,排除手术、肿瘤病史及其他影响BMD的疾病患者。选取腰2椎体中间层面测量3个感兴趣区的钙-水、HAP-水、HAP-Fat,并取其平均值。按同样标准选择同期在我院接受腰椎DXA检查的成年女性117例作为对照组,年龄21~88岁,纳入标准和年龄分组同观察组。对观察组中各年龄的测量结果进行分析比较。对年龄与HAP-水密度、各年龄段L2椎体平均HAP-水密度与DXA测得的BMD进行相关分析。结果 观察组中各年龄段间差异有统计学意义(P<0.05)。≤39岁者年龄与HAP-水密度呈正相关(r=0.458,P<0.05),≥40岁者年龄与HAP-水密度呈负相关(r=-0.745,P<0.05)。各年龄段L2椎体HAP-水密度与DXA测得的BMD值呈正相关(r=0.516,P<0.05)。结论 能谱CT物质定量分析功能能反映椎体骨质密度变化,其测量结果与DXA测量结果有一定相关性,均与年龄相关。Objective To explore the possibility of HAP-water,calcium-water,and HAP-Fat-based material imaging to measure bone mineral density by comparing with dual energy X-ray absorptiometry using the quantitative analysis function of spectroscopic CT.Methods A total of 146 healthy females who underwent upper abdominal CT examination with spectral CT at Shanghai Songjiang District Central Hospital were enrolled as the research group.Those patients with surgery,trauma,tumor or other diseases affecting BMD were excluded.Calcium-water,HAP-water,and HAP-Fat were measured in three ROI at the intermediate level of the lumbar 2 vertebral body,and the average value was taken.According to the same criteria,117 adult women,aged 21~88 years,who underwent lumbar spine DXA examination in our hospital,were selected as the control group.The inclusion criteria and age groups were the same as the study group.The measurement results of each age in the study group were analyzed and compared.Age and HAP-water density,and the average HAP-water density of L2 vertebral body in each age group and BMD measured by DXA were analyzed.Results There were statistically significant differences between the age groups in the study group.The age of≤39 years old was positively correlated with HAP-water density(r=0.458,P<0.05),and the age of≥40 years old was negatively correlated with HAP-water density(r=-0.745,P<0.05).The HAP-water density of the L2 vertebral body was positively correlated with the BMD value measured by DXA in each age group(r=0.516,P<0.05).Conclusion Material decomposition images of spectral CT can reflect vertebral bone changes,and its measurement results have a certain correlation with DXA measurement results,both of which are related to age.
